Miner stepped down off plant ladder and rolled ankle. Miner was using three points of contact and was wearing full top steel toe boots. Ground conditions were smooth and well kept.

The injured employee was blowing dust off the plant with compressed air, while blowing the dust off they broke protocol and inserted the air nozzle behind a guard on the under side of a tail pulley. The pulley yanked the nozzle from the employees hands cutting their palm.
The mine foreman was cutting a damaged piece of belting out with a utility knife, EE placed EE's hand in a bad position while cutting and slit EE's hand. EE drove to a local clinic where they gave EE 4 stitches. EE returned to work, total time away from work approx. 4 hours.
Injured Employee set a piece of welding equipment up on the frame of a machine and was struck when EE ducked under the frame and the equipment fell down. EE's hard hat stopped most of the damage, but it was pushed off and EE received a large gash on the back of head. EE was taken in for treatment. EE missed two days and has been released for work.
Employee reported that he was sweeping a sample box through product coming off conveyor belt to collect aggregate material when he felt a sharp pain in his right shoulder.

Employee was taking the counterweight cover out of the cone crusher. When he pulled back to get it loose, his hand hit a piece of brass bushing, and it peeled away some of the skin on his hand.

While employee was going down ladder to get off loader, he twisted his left ankle. He had medical evaluation later that day, but no medical treatment. Pain symptoms persisted, so he was re-evaluated medically on 8/12/13, and referred for physical therapy treatment.

Employee went to the doctor on 3/12/2011 because he believed he got a piece of foreign material in his eye on 3/11/2011. He notified the employer on 3/14/2011.

Employee collapsed while loading bulk explosives into boreholes in preparation for a production blast. CPR was started immediately by co-workers. Employee was taken by ambulance to local hospital and pronounced dead approx 1 hour later. Autopsy results are currently pending. Co-workers stated employee appeared normal leading up to the incident and showed no signs of distress.
Employee was finishing up work on loader and putting tools away. Experienced sharp pain in middle back under shoulder blade. Employee states he was twisting and reaching in an awkward position.

Employee was checking the anti-freeze on his haul truck. The night of 2/8/05 the weather caused frost to bank the quarry. As ee was climbing down, the step had a build up of frost on it and he lost his footing / balance and fell onto concrete. He was wearing a safety helmet and reflector vest.

Employee was helping to fold stacking conveyor, standing on a shroud on the edge of the conveyer. Conveyor shook, causing employee to fall to the ground.
