Raw Grind Operator was shoveling fine from rock run into the secondary crusher hopper. Operator felt a sharp pain in lower back while lifting a shovel of gypsum fines.

EE was changing C-1 belt in the primary crusher pit. He attached a 1.5 ton chain hoist to a I beam to raise the new belt. The hook on the chain hoist was positioned on the I beam at an angle. When the mechanic pulled the slack out of the hoist, the head of the hoist turned allowing hoist to fall hitting EE in head & face. He sustained 2 cuts on upper lip requiring 5 sutures.

Maintenance employees removing the right rear leaf spring from a lube truck. Spring had been unbolted and was being lifted out by 3 employees. The employee at the edge of the truck was told to let go. The spring rolled on its side, pinching another employees small finger causing a small cut requiring two stiches.

Mechanic was changing oil on a haul truck. You have to let down the belly pan to gain access to the filter. The pan was full of mud and was heavier than normal, falling and striking the mechanics wrist and bending it backward.

EE was cleaning under the Pan Feeder at the Screen Plant. At break time he left to walk up to control room. As he walked down the 2 steps in this area, he stepped onto the expanded metal walkway. Walkway had small depression & he stepped in it, resulting in him twisting his ankle & falling down on walkway. Xrays were negative. Dx was ankle sprain & foot sprain.

DRILL CENTRALIZER MALFUNCTIONED ALLOWING DRILL STEEL TO DROP. THE DRILL STEEL WAS BEING LIFTED AND ALIGNED INTO THE CENTRALIZER WHEN IT SLIPPED THROUGH THE CENTRALIZER AND A RESTRAINING ROPE.

EMPLOYEE WAS DRIVING THE COMPANY 3/4 TON PICK UP FROM SHOP AREA TO THE R07 MINE AREA. ON THE 1ST CURVE OF THE MINE HAUL ROAD, HE LOST CONTROL OF THE TRUCK. THE TRUCK SLID SIDEWAYS & HIT THE BERM. THE TRUCK THEN ROLLED SEVERAL TIMES & THE EE WAS THROWN FROM THE VEHICLE. EE WALKED AWAY BUT WAS TAKEN BY AMBULANCE TO THE HOSPITAL.

Employee was cribbing up, off road truck with crossties, to remove tires & wheels, one tie about 3-4 feet long fell over, employee bent over to pick up, tie was wet, gloves were wet & it slipped in hands, causing employee to grab for it. When he caught the tie he felt a twinge in his back.

EE SHUT DOWN THE GENERATOR FOR THE END OF THE SHIFT. AS HE WALKED OUT OF THE GENERATOR TRAILER, HE STEPPED ON TO THE PLATFORM AT THE TOP OF THE STAIRS. THE PLATFORM WAS COVERED WITH ABOUT 1"OF SNOW. AS EE STEPPED ON THE SOLID METAL SECTION OF THE PLATFORM, HIS FEET WENT OUT FROM UNDER HIM AND HE FELL DOWN, STRUCK HIS ARM WHEN HE LANDED AND THEN HE SLID DOWN THE STAIRS.

EE STATED THAT BACK ON 11-11-00 WHILE OPERATING THE 988 LOADER HIS BACK BEGAN TO BOTHER HIM. HE DID NOT THINK ANYTHING OF IT UNTIL THE 17TH IT STILL BOTHERED HIM AND HE REPORTED IT TO OTHER. T HE MOVMENT OF THE LOADER AND UNEVEN ROAD APPARENTLY CREATED THE PROBLEM.

EMPLOYEE WAS DRIVING THE LOADER OVER ROUGHT TERRAIN AND THE BOUNCING SEEMED TO STRAIN HIS BACK. HE FELT A TINGLING IN HIS LEFT ARM. TOOK INDIVIDUAL TO DOCTOR ON 9/26; THIS STARTED AS A FIRST- AID CASE BUT HAS SINCE CHANGED TO A RECORDABLE INCIDENT.
