It was raining. EE was pressing bucket pin out with 70 ton press and a track pin. Press was at a slight angle. Another mechanic was hitting outside boss, EE was sitting beside press gear when it pitched sideways and track pin hit him on upper lip, leaving a hole. EE had to have 7 stitches on his upper lip.

Worker was walking from one side of mine to the other when the wind blew a foreign object into his eye. Was was wearing safety glasses. Worker went to urgent care but the extraction was unsuccessful so they referred him to the ER.
The employee was rotating gears to change timing and pinched his fingers between the bar and the frame. At first, the employee decided he was okay but later decided to see a doctor.

EE REPORTED IN mARCH A BACK INJURY EE SAYS EE SUSTAINED CHANGING THE BLADES ON A ROAD GRADER.
EE TURNED AROUND TO GO DOWN THE POWER SCREEN LOADER, SLIPPED, FELL HURTING HAND AND WRIST.
The ee cut his right index finger on a screen while changing it.
THE EE WAS GRINDING ON METAL AND GOT A PIECE OF METAL UNDER HIS GLASSES AND INTO HIS EYE.
EE WAS BUILDING BERMS ON A STOCKPILE. HE SAYS HE HAD HIS LOADER BUCKET 1/2 FULL WHEN HE DROVE BACK OVER A DIVOT HE CREATED CAUSING THE BUCKET TO HIT THE GROUND WHICH HURT HIS NECK. THE INCIDEN T OCCURRED ON MONDAY & WAS NOT REPORTED UNTIL FRIDAY.
THE EE WAS WORKING ON A CONVEYER WHEN HE RAISED UP HITTING THE BACK OF HIS HEAD ON THE CONVEYER.
THE EE WAS TAKING OFF A BOLT ON A CONVEYER BRACKET, THE BRACKET FELL AND CUT HIS FINGER.
EE WAS CHANGING THE SCREENS TO A SCREEN DECK WITH AN IMPACT WRENCH. HE WAS SWEATING & HE WIPED HIS BROW. HE FELT SOMETHING ON HIS EYELASH, AND WHEN HE WIPED HIS LASHES, HE FELT SOMETHING IN HIS EYE.
THE EMPLOYEE WAS TRYING TO CLIMB A SMALL LADDER ON OUR BLENDER MACHINE, SLIPPED ON SOME SNOW AND MUD AND HIT HIS CHEST ON THE FRAMEWORK.
THE EE WAS BIT ON THE LEFT HAND BY A SMALL BLACK SPIDER. HIS HAND STARTED TO TINGLE AND SWELL SO HE WAS TAKEN TO THE DOCTOR.
THE DRIVER TRIED TO MAKE A TURN AT A HIGH RATE OF SPEED TURNING, HIS HAUL TRUCK OVER. INITIALLY THE EE HAD A BRUISED HIP ON 10-3 HE SAW THE DR AND COMPLAIN ID OF BACK PAIN.
EE WAS REPLACING THE METAL GRATES ALONG THE SCALE AREA & FELT A PAIN IN HIS BACK.
THE EE WAS CLIMBING DOWN A STAIR/LADDER FROM A SCREEN DECK AND STEPPED ON A DIRT CLOD, SPRAINING HIS ANKLE.
EE WAS WORKING AT THE WELDING TODDLE AT THE SITE AND A BEE STUNG HIM ON THE HAND HIS HAND SWELLED TO DOUBLE THE SIZE AND WAS TAKEN TO THE DR THE NEXT DAY.
THE MINER WAS USING A HAMMER TO KNOCK OFF A LOADER TOOTH & THE HAMMER FLEW UP & HIT THE WORKER ON THE UPPER CHEEK.
THE EE WAS CUTTING A SECTION OF BELTING WITH A BOX CUTTER AND THE KNIFE SLIPPED CUTTING THE TOP OF HIS LT. HAND.
THE EE SLIPPED ON SOME ICE WHILE HE WAS THAWING OUT THE SCREEN DECK AND FELL ON THE PROPANE TANKHE WAS USING.
THE HEAVY EQUIPMENT MECHANIC STEPPED OFF AN EXCAVATOR ONTO A ROCK AND TWISTED HIS (L) ANKLE.
THE EE WAS TRYING TO DISLODGE OVERSIZED ROCK IN A CHUTE. HE USED A SHOVEL AND WHEN THE ROCKS DISLODGED ONE CAME DOWN AND HIT HIS FINGER.
THE EMPLOYEE WAS TOSSING A HAMMER OUT OF THE SCREEN DECK HE WAS IN AND THE HAMMER BOUNCED BACK STRIKING THE EMPLOYEE IN THE FACE.
A CONVEYER WAS BEING WORKED ON AND WHEN THE EMPLOYEE BRUSHED AGAINST THE ELECTRICAL CAUSING SPARKING AND POPPING WHICH CREATED A BURN ON THE EMPLOYEE'S ARM. A REVIEW OF THE LOCKOUT REVEALED THAT SEVERAL STITCHES WERE NOT ACCURATE. THEY HAVE CORRECTED SINCE.
THE EMPLOYEE WAS TIGHTENING A BOLT WHEN THE WRENCH SLIPPED OFF THE BOLT STRIKING HIS UPPER LIP.
THE EE WAS MOVING PIPE AND CUTTING PIPE TO REINSTALL A WATER LINE. AT THE END OF THE DAY HE SAID HIS BACKING SORE. WAS SENT TO THE DR WHEN BACK PAIN PERSISTED. HE WAS RETURNED TO REGULAR DUTY AFTER VISIT TO DR.
EE WAS STEPPING DOWN FROM HIS LOADER IN THE PIT AREA. WHEN HE STEPPED TO THE GROUND WITH HIS R LEG, HIS ANKLE FOLDED UNDER HIM & HIS KNEE BENT OUT. INITIAL ASSESMENT IS A KNEE STRAIN
THE CONTRACTOR'S EE WAS PULLING ROPE & WIER NEXTTO A PORTABLE CONTROL PANEL THAT WAS BEING PLACED AT THE EDGE OF THE CONCRETE PAD. ONE SIDE OF THE PORTABLE'S STAND LEGS WERE NOT ON THE CONCRET E PAD & IT WAS NOT OBSERVED HOW THE ELECTRICAL PANEL FELL OVER, BUT IT DID KNOCKING THE CONTRACTOR EE TO THE GROUND
THE EE WAS REMOVING A TRAILER HITCH FROM A PICKUP WHEN THE HITCH AND TRAILER ROLLED FORWARD PINNING THE EMPLOYEE'S FINGER AGAINST THE PICKUP.
EMPLOYEE SAYS HE WAS CARRYING A 10' SECTION PVC PIPE AND FELT A POP IN HIS RIGHT SHOULER. THE COMPANY FIRST FOUND OUT ABOUT THE INJURY FROM A DR'S REPORT DELIVERDD 8-23-2000.
AN 8 FT POLE IS USED TO BREAK UP TOP SOIL INSIDE A FEED HOPPER FROM AN ELEVATED CATWALK. THE EE IS NOT USED TO THE ACTIVITY AND DEVELOPED A MINOR STRAIN TO THE R WRIST.
THE EMPLOYEE WAS TAKING APART A CONVEYOR BRAKE WHEN THE WRENCH HE WAS USING SLIPPED AND HE HIT HIS KNUCKLE ON THE BRAKE.
