A unintentional roof fall occurred on the #11 belt at spad # 5935. The belt line was the only equipment involved in the incident. No injuries occurred. The fall measured 24' wide 28' long and 6' high.

Employee was climbing up the steps on the 980H loader. EE reached for the door handle, the door handle was slick from the rain causing hand to slip off the handle as the door opened. This caused ee to fall off the loader hurting back, shoulder, neck and arm.
A pillar fall occurred on the 003 MMU 60' in by SS#8673 in the #2 entry resulting in 2 MRS units being trapped. The fall occurred on pillar section in pillar line. Continuation of the pillar fall was the cause.

Man trip employee pulled over to let another man trip go by and the employee had part of his right foot sticking out of the ride. His foot got caught on the rib resulting in multiple fractures.
On 11/20/14 a roof fall occurred on the 003 MMU, #5 entry, 1 break in by SS#6763 resulting in 2 MRS units being trapped. Fall occurred on pillar section in pillar line. #3 MRS equipment failure was the cause of the fall.

Employee was bolting in #6 entry, putting up the last row of bolts in the face. The hole was drilled and the employee was putting the glue in the hole when a piece of rock fell glancing him on the back of the head resulting in a laceration.

Employee was placing crib blocks under a diesel engine that was being held by a forklift. When he bent over to put the crib block in place he struck his head on one of the forks resulting in a laceration requiring staples in his head.

Employee stated he was replacing the main jack on ATRS roof bolter FB#21. The ATRS was secured to mine roof by chains, the main jack was secured to drill by bottom pin. Employee started machine at operator's station which position on end of ATRS bar, the ATRS bar fell striking him on front of hard hat and right shoulder.

Employee was putting bottom roller in on #1 belt when roller came out of hanger causing employee to fall backwards hitting mantrip. Employee felt pain in his lower back on the left side and down his left leg.
Employee was on his knees next to the miner at the control panel. Mechanic was working inside the contact panel checking voltage from the master panel to the contact panel. The master control panel and contact panel was swung out. When employee began to rise from his knees he inadvertently laid his right hand on the swing out portion of the contact panel which was energized.
