EE stated he was in the process of examining the primary sampler for the clean coal loadout, while moving the metal that was next to the sampler cutter guard, the metal fell pinning him against the floor, causing fractures to both legs and his left hip.

EMPLOYEE WAS HEATING A BRACKET THAT HOLDS THE DUST BOX ON A 320 CAT EXCAVATOR WITH A DRILL ATTACHMENT. AFTER HEATING THE BRACKET, EE WAS ATTEMPTING TO STRAIGHTEN THE BRACKET USING A LONG PIECE OF PIPE TO APPLY PRESSURE TO THE BRACKET. EE WAS STANDING IN A MUDDY AREA THAT HAD LOOSE MATERIAL. THE PIPE SLIPPED FROM THE BRACKET & EE SLIPPED, FALLING ON HIS LT. LOWER LEG BREAKING IT.

EE STATED HE WAS CUTTING A PIECE OF METAL FROM INSIDE THE RAW COAL CHUTE, WHEN THE PIECE OF METAL WAS FREE, IT FELL STRIKING HIM ON HIS RIGHT FOOT CAUSING A LACERATION AND FRACTURE.

EMPLOYEE WAS STANDING ON A 8' STEP LADDER, HITTING ON SIDE OF CHUTE WITH SLEDGE HAMMER. ONE OF THE LEGS OF THE LADDER SPLIT CAUSING LADDER & EMPLOYEE TO FALL. SEEN DOCTOR 10/28/01 X-RAYED FRAC TURE OF WRIST.

EMPLOYEE STEPPED ONTO AN OUTRIGGER OF A CRANE, HIS LEFT FOOT SLIPPED AND HE FELL APPROX. 4' TO THE GROUND. EMPLOYEE LANDED ON HIS BACK, HE COMPLAINED OF PAIN TO LOWER BACK, AND RIGHT WRIST. EM PLOYEE SENT TO ARH FOR EXAM, X-RAYS NEG. FOR ANY FRACTURES, EMPLOYEE MISSED 4 DAYS OF WORK.

EE WAS DESCENDING EMBANKMENT TO GET A SLURRY SAMPLE. HE SLIPPED ON LOOSE ROCK, FELL AND INJURED HIS RIGHT SHOULDER.

WE HAD PUT A ROLL OF BOLT ONTO THE PLANT FEED BELT LINE. EMPLOYEE WAS BEHIND THE TAIL ROLLER. THE BOLT SLID AROUND, PINNING EMPLOYEES LEG BETWEEN THE ANGLE IRON RAIL AND THE BELT.

EMPLOYEE WAS SHOVELING COAL FROM A SPILL. EMPLOYEE LIFTED A SHOVEL FULL AND TURNED, THE WENT TO HIS KNEES HE WAS HELPED UP BY ANOTHER EMPLOYEE.
