An unintentional roof fall occurred in the KY Ridge Mains in the #6 entry between Xcut 26 and 27. The fall is approximately 40' long, 20 ' wide and 7-8' height. The fall does not effect the passage of miners or the ventilation system for this area. This area is part of an inactive area of the mine on the intake side and is examined weekly.

Injury said that he was trying to exit the scoop & his foot got stuck under the starting box. He twisted his knee trying to get out. He said if the foreman would take it easy on him, he wouldn't go outside and fill out an acc. rpt or go home and seek medical attention. There were two witness besides the foreman to this statement.

The miner was going underground, operating a scoop. The scoop hit the outside canopy, the miner slid over and hit his neck.

The miner was trying to access the feeder. He was assisted in taking the guard off the pick breaker. As both miners were raising the guard, one miner screamed with pain.

Employee was cleaning up debris on the bottom floor. A ceramic lined chute was being removed 3 floors above. A 1/2" X 4" X 6" pc of ceramic fell from the chute into the machinery well, bouncing from side to side. The employee app 30' away from the barricaded area was struck on the rt ring finger.

Two men were tied off in the basket of a manlift approx. 15' in the air. They went to swing the basket in closer to the concrete forms on the stacking tube. The basket moved quickly causing injured to hit his lower back against the rail of the basket.

EE WAS RIDING IN A MANTRIP THAT WAS DEAD. IN AN ATTEMPT TO GET THE UNIT TO THE CHARGING STATION, HE HAD A SCOOP PUSH THE UNIT. IT THEN WEDGED AND CAUSED THE EE TO HIT THE MINE ROOF.
