Showing all 6EE's were replacing troughing roll assemblies from under a screen deck, EE was using a 2ft pry bar to flip a roll out for replacement, as the roll flipped out the frame of the roll frame came down on the EE's right index finger.
Employee's foot slipped off the CAT 374 excavator track while dismounting during the morning startup maintenance. Employee's right wrist struck the track as they fell causing a fracture of the right wrist. Employee stated that their boots had mud buildup causing loss of traction.
Prying a large stone that was jammed on a feeder, stone shifted quickly and caused the pry bar to strike right hand causing injury to right ring finger.
Employee was riding in the maintenance truck to go inspect the area around the jaw crusher for snow. Upon exiting the truck they stepped on some ice and slipped but did not fall down. Employee has a left lateral tibial plateau fracture.
EE was installing flashing on the 206 scalping screen, EE was holding the flashing in place on the screen with rt. hand while using lt. hand to operate the drill. The 1/2" drill bit came through the flashing, caught EE's glove, went through it and into right middle finger. Dermabond applied to wound.
