Employee was riding in the maintenance truck to go inspect the area around the jaw crusher for snow. Upon exiting the truck they stepped on some ice and slipped but did not fall down. Employee has a left lateral tibial plateau fracture.

Employee was reaching for gloves left on grownd when hit by blocking. Employee has hematoma-contusion to left lower front of leg. Employee on light duty work, no loss of time.

EE WELDING, LATER IN THE DAY EYE STARTED BOTHERING HIM AND CONDITION PERSISTED. SOUGHT MEDICAT ATTENTION ON 5-5-00, FOUND FOREIGN OBJECT IN EYE, WHICH WAS REMOVED 5-6-00 BY OPTOMETRIST. NO PER MANENT DAMAGE TO EYE.
