The miner was using a welding torch when the oxygen hose burst and ignited. The fire burned through the miners' jacket and shirt resulting in a burn to EE's stomach.

While installing a screen on the middle deck, employee had hand on bottom of screen when the screen shifted and caught hand and fingers between screen and bottom of deck. Employee was treated and received stitches. Employee returned to work the following day with restrictions.

Miner was performing housekeeping duties under a steel structure that supports a stacker/conveyor belt. As EE crouched down, the hard hat slipped off EE's head. When EE stood back up, EE hit head on the low part of the steel structure. EE was transported to urgent care by a co-worker. EE received 3 staples to close the cut on EE's head.
Employee tried to fix a skirt board rubber while belt was in operation. Employee did not contact the Plant operator as required for Lockout. Injured Thumb- right hand

Employee was carrying a chain to the primary feeder to wrap around a rock. The employee tripped and fell on same level. EE caught the fall using right wrist causing EE's wrist to fracture.

The employee dropped something while using a torch. EE bent over to pick up the item and the flame from the torch burnt EE's arm. EE states should have turned the torch off before retrieving item. Employee has a 2nd degree burn on right arm. No lost time from work.

Employee was welding within the primary crusher when a piece of hot material fell into the top of a boot and burned their ankle and foot. Employee went to the emergency room and was transported to a burn unit. Diagnosed the injury as third degree burns on their right foot. Employee has not returned to work.

Miner was operating a haul truck which was being loaded. A large stone was placed in bed which caused the truck to shake. Miner felt sharp pain in back and neck.

Employee was cutting a ACSR cable with a band saw. EE was holding the cable in EE's left hand, saw in the right. After the cut was made, the blade stopped and came to a rest on top of EE's left hand, causing a cut requiring four stitches. The employee finished EE's shift after the cut, got the stitches and returned for work at the normal starting time the next day.
Employee lost control of vehicle while descending the ramp. EE hit a rock which stopped the vehicle. EE left hand hit dashboard on impact.

Employee was cutting a bolt with a torch. A piece of hot steel landed on his right pinky finger. Employee waited to report and get medical attention, the finger swelled, blistered then became infected.

The employee believed he had lowered the dump body on the 35 ton haul truck. He hit the conveyor, which fell onto the back of the truck. When the truck hit the conveyor the truck tilted on back wheels until it broke free from the convoyer causing the truck to land back on all 4 tires.

Employees were cleaning screen house spillage. They opened a floor panel to drop stone into bins. The injured employee walked up onto the stone pile which started to slide. Employee could not stop himself from sliding. The stone and employee slid into the the bin. He fell approx 20 ft onto stone in bin.

EE WAS CONDUCTING MAINTENANCE ON A CRUSHER. WHEN THE GROUP WAS REPOSITIONING THE CRUSHER LID, THE LIFTING CABLE SHIFTED, CAUSING THE LID TO SHIFT. EE'S HAND WAS CAUGHT BETWEEN THE LID AND A PIECE OF METAL.
