While working on a crusher, the miner slipped and fell onto EE's shoulder causing pain.

The miner was cutting a belt and accidentally cut left wrist.

The miner was using a welding torch when the oxygen hose burst and ignited. The fire burned through the miners' jacket and shirt resulting in a burn to EE's stomach.

Opened inspection door to check adjustment, turned rotor when I felt something pop in right shoulder

While installing a screen on the middle deck, employee had hand on bottom of screen when the screen shifted and caught hand and fingers between screen and bottom of deck. Employee was treated and received stitches. Employee returned to work the following day with restrictions.

Miner was performing housekeeping duties under a steel structure that supports a stacker/conveyor belt. As EE crouched down, the hard hat slipped off EE's head. When EE stood back up, EE hit head on the low part of the steel structure. EE was transported to urgent care by a co-worker. EE received 3 staples to close the cut on EE's head.

Employee was walking up a hill when some 8" rocks came down striking their foot. Employee was wearing steel toed boots, rock hit in lace area.

Employee tried to fix a skirt board rubber while belt was in operation. Employee did not contact the Plant operator as required for Lockout. Injured Thumb- right hand

Employee was carrying a chain to the primary feeder to wrap around a rock. The employee tripped and fell on same level. EE caught the fall using right wrist causing EE's wrist to fracture.

The employee dropped something while using a torch. EE bent over to pick up the item and the flame from the torch burnt EE's arm. EE states should have turned the torch off before retrieving item. Employee has a 2nd degree burn on right arm. No lost time from work.

Employee was working on the crusher. EE went to grab a tool when EE tripped over frozen mud. Tried to catch self on nearby truck, but fell anyways pushing EE's arm behind self.

Slipped and fell on ground while walking and hit head. Received staples in the head and is able to return to work full duty.

Employee was welding within the primary crusher when a piece of hot material fell into the top of a boot and burned their ankle and foot. Employee went to the emergency room and was transported to a burn unit. Diagnosed the injury as third degree burns on their right foot. Employee has not returned to work.

Miner was operating a haul truck which was being loaded. A large stone was placed in bed which caused the truck to shake. Miner felt sharp pain in back and neck.

Employee was cutting a ACSR cable with a band saw. EE was holding the cable in EE's left hand, saw in the right. After the cut was made, the blade stopped and came to a rest on top of EE's left hand, causing a cut requiring four stitches. The employee finished EE's shift after the cut, got the stitches and returned for work at the normal starting time the next day.

Employee lost control of vehicle while descending the ramp. EE hit a rock which stopped the vehicle. EE left hand hit dashboard on impact.

Employee slipped on the crusher framework and fell onto back.

Employee's left hand became entrapped between the head pulley and belt cleaner while inspecting a chute on the Screening belt #3.

Was walking around stationary vehicle on bench.

Employee's annual audiogram and follow-up audiogram indicate a standard threshold shift hearing loss.

Loose stone was not removed and/or secured prior to entering the crusher. Employee was stuck and trapped by 2 rocks that fell from an elevated position. Employee felt that the rocks were not going to move prior to the accident.

Employee was jumping an excavator when the battery exploded. Two Employees were injured.

Employee was operating a water wagon (E21) when he turned into a passing haul truck. Employee stated that he did not see the haul truck prior to turning.

While grinding, the grinder jumped and hit the employees hand causing a laceration that need stitches.

A piece of grade blade fell, as the employee reached for it he felt a pain in his side and back.

Employee was lifting a pip-men out of the primary crusher when the worn/greased shaft slide out of its position, falling onto the crane's operator station.

Employee was changing a tire on a light duty pickup truck when he twisted his ankle.

Employee was pulling/hanging an electrical cable when he came in contact with the two copper wires. There was a piece of the outer jacket that was worn out which the employee did not notice.

Employee was removing bolts inside the crusher when one of the impellers moved & knocked employee back into the metal bar.

Employee was assisting the disassembly of a crane when the outrigger lock lever struck his right index finger.

The employee believed he had lowered the dump body on the 35 ton haul truck. He hit the conveyor, which fell onto the back of the truck. When the truck hit the conveyor the truck tilted on back wheels until it broke free from the convoyer causing the truck to land back on all 4 tires.

Employees were cleaning screen house spillage. They opened a floor panel to drop stone into bins. The injured employee walked up onto the stone pile which started to slide. Employee could not stop himself from sliding. The stone and employee slid into the the bin. He fell approx 20 ft onto stone in bin.

Employee was repairing a screen barring when the wrench slipped causing the employee's finger to get jammed between the wrench and barring.

While climbing a ladder he slipped and fell.

EE WAS PERFORMING MAINTENANCE ON A HAUL TRUCK. WHEN HE WENT TO REMOVE HIS HAND, HIS FINGER BECAME CAUGHT ON THE HEATER BOX. HIS FINGER WAS CUT WHEN HE TRIED TO REMOVE IT.

EE WAS CONDUCTING MAINTENANCE ON A CRUSHER. WHEN THE GROUP WAS REPOSITIONING THE CRUSHER LID, THE LIFTING CABLE SHIFTED, CAUSING THE LID TO SHIFT. EE'S HAND WAS CAUGHT BETWEEN THE LID AND A PIECE OF METAL.

Ee was using a cutting torch when the torch back-fired due to grease splotter.

EMPLOYEE WAS WORKING TO MOVE A ROLLER WITH A PRY BAR. DURING THE MOVE, THE ROLLER (NON MECHANICAL/ELECTRIC) ROLLED BACK CAUSING THE EMPLOYEE TO LOSE GRIP OF THE PRY BAR. THE BAR FELL ONTO HIS LEFT FOOT.
