Fall to the walkway or working surface

While working on a crusher, the miner slipped and fell onto EE's shoulder causing pain.

Contact with hot objects or substances

The miner was using a welding torch when the oxygen hose burst and ignited. The fire burned through the miners' jacket and shirt resulting in a burn to EE's stomach.

Employee was working on the crusher. EE went to grab a tool when EE tripped over frozen mud. Tried to catch self on nearby truck, but fell anyways pushing EE's arm behind self.

Contact with hot objects or substances

Employee was welding within the primary crusher when a piece of hot material fell into the top of a boot and burned their ankle and foot. Employee went to the emergency room and was transported to a burn unit. Diagnosed the injury as third degree burns on their right foot. Employee has not returned to work.

Employee lost control of vehicle while descending the ramp. EE hit a rock which stopped the vehicle. EE left hand hit dashboard on impact.

Employee was cutting a bolt with a torch. A piece of hot steel landed on his right pinky finger. Employee waited to report and get medical attention, the finger swelled, blistered then became infected.

Loose stone was not removed and/or secured prior to entering the crusher. Employee was stuck and trapped by 2 rocks that fell from an elevated position. Employee felt that the rocks were not going to move prior to the accident.

Lifting steel plate - performing routine maintenance, when the plate fell crushing finger.

Employee was removing bolts inside the crusher when one of the impellers moved & knocked employee back into the metal bar.

The employee believed he had lowered the dump body on the 35 ton haul truck. He hit the conveyor, which fell onto the back of the truck. When the truck hit the conveyor the truck tilted on back wheels until it broke free from the convoyer causing the truck to land back on all 4 tires.

Fall from piled material

Employees were cleaning screen house spillage. They opened a floor panel to drop stone into bins. The injured employee walked up onto the stone pile which started to slide. Employee could not stop himself from sliding. The stone and employee slid into the the bin. He fell approx 20 ft onto stone in bin.

EMPLOYEE WAS WORKING TO MOVE A ROLLER WITH A PRY BAR. DURING THE MOVE, THE ROLLER (NON MECHANICAL/ELECTRIC) ROLLED BACK CAUSING THE EMPLOYEE TO LOSE GRIP OF THE PRY BAR. THE BAR FELL ONTO HIS LEFT FOOT.

EE WELDER SLIPPED ON A CONVEYOR BELT. THEN FELL INTO A CROSS BEAM HITTING HIS HEAD, THEN FELL BACKWARDS LANDING ON HIS BACK.

EE WAS EXITING HIS HAUL TRUCK. WHEN HE REACHED THE BOTTOM OF THE LADDER. HE STEPPED ONTO A LARGE STONE THAT MOVED THIS CAUSING HIM 6O TWIST HIS ANKLE.

EE WAS SWEEPING AND SHOVELING STOVE FROM UNDER BIN CLAIMS TO HAVE HURT HIS NECK AND SHOULDER.
