The miner was cutting a belt and accidentally cut left wrist.

Opened inspection door to check adjustment, turned rotor when I felt something pop in right shoulder

Employee was carrying a chain to the primary feeder to wrap around a rock. The employee tripped and fell on same level. EE caught the fall using right wrist causing EE's wrist to fracture.

Employee was removing a guard when EE received a laceration on right hand from a sharp edge.

Employee was lifting a pip-men out of the primary crusher when the worn/greased shaft slide out of its position, falling onto the crane's operator station.

Employee was pulling/hanging an electrical cable when he came in contact with the two copper wires. There was a piece of the outer jacket that was worn out which the employee did not notice.

Employee twirled his hand on the engine pull start

Employee was repairing a screen barring when the wrench slipped causing the employee's finger to get jammed between the wrench and barring.

EE WHILE CLEARING A PILE OF STONE HIT SOLID ROCKWHILE USING A PICK AXE.

EE WAS CLEANING/REPAIRING THE INSIDE OF A LARGE BIN WHEN A LARGE AMOUNT OF DEBRIS FELL ON HIM. EE WAS PULLED FROM BIN DISORIENTED - (EE STATES HE LOST CONSCIOUSNESS FOR A MINUTE). EE DECLINED MEDICAL TREATMENT UNTIL 8/25/03.

EMPLOYEE WAS REACHING FOR AN IMPACT GUN WHEN A ROCK FELL FROM AN UNKNOWN LOCATION AND STRUCK A PRY BAR THAT WAS THROWN INTO EE'S LEFT BICEPT.

EE WAS DESCENDING THE STAIRS OF THE 84 CRUSHER BUILDING WHEN HE SLIPPED - TWISTING HIS ANKLE.

WALKING DOWN EMBANKMENT TRIP, LOST FOOTING FELL STRIKING HEAD ON ROCK AND LEFT ARM ALSO FACE LOST TWO TEETH 34 STITCHES.
