A miner had unknowingly entered the screening area to assist other miners who had completed a conveyor repair. The injured miner got his left hand pinched in between the conveyor belt and tail pulley. Injuries sustained include broken wrist and cuts on hand and arm. A full accident report with all details will be attached to this online form if the opportunity allows.
Lifting an oxygen tank
EE was operating a CASE 1845C. A weld on the skid-steer lift pin failed and broke. The lift arm moved from normal position and contacted the machine cab and caused the cab to shift to the right side. The lower portion of the cab came in contact with EE's boot and pinned it between the foot-rest and the cab. First aid was given by applying an ice pack.
At approx. 10:20 AM EE went to go to the restroom. She opened office door to go out & when she stepped on the grating her foot slipped causing her to fall. Her left foot slipped out & went under the step. There was a little ice on the shaddy part of the grating. The next morning her foot was still hurting so she was sent to the Dr. & Dr. said foot was sprained & to stay off it.
A rock was lodged in the crusher. A chain was put on rock to pull it out. Loader operator asked injured if all chains were on. He replied 2 chains were in loader bucket. Loder operator didn't know injured had climbed into bucket. He lifted loader bucket Appx 8 feet and began to dump chains out. Injured fell out of bucket landing on feet & falling down. Chipped heel bone.
Was going up ramp & engine stalled out. Lost air to brake system, truck hit berm and tipped over. Bent door, broke seat & battery box restraint. EE was asked if he was ok, said he was fine. No visual injury noted. Asked later if he needed to be checked out. EE said no he might be sore, bump on hand & battery fell on leg. RTW 11/2/07. Fine till noon & complained of back hurting.
A piece of steel hit and lodged in right shoulder. Went to emergency room for removal and had three stitches placed. returned to work afterwords the same day.
Employee was up on Loader and was getting ready to get off and tripped over his own foot falling to the ground. Has Bruised or slight cracked rib.
ICED OVER MUD HOLE BROKE THROUGH WHEN EMPLOYEE STEPPED ON IT AS HE WAS WALKING THROUGH WORK AREA.
EMPLOYEE WAS WORKING AND RAN TO SHUT OFF CONVEYOR BELT; STEPPED ON A ROCK AND TWISTED RIGHT ANKLE. ANKLE SWELLED AND TURNED BLACK AND BLUE, ALOT OF PAIN.
