Fall down stairs

At approx. 10:20 AM EE went to go to the restroom. She opened office door to go out & when she stepped on the grating her foot slipped causing her to fall. Her left foot slipped out & went under the step. There was a little ice on the shaddy part of the grating. The next morning her foot was still hurting so she was sent to the Dr. & Dr. said foot was sprained & to stay off it.

Fall from machine

A rock was lodged in the crusher. A chain was put on rock to pull it out. Loader operator asked injured if all chains were on. He replied 2 chains were in loader bucket. Loder operator didn't know injured had climbed into bucket. He lifted loader bucket Appx 8 feet and began to dump chains out. Injured fell out of bucket landing on feet & falling down. Chipped heel bone.

Struck against stationary object

A piece of steel hit and lodged in right shoulder. Went to emergency room for removal and had three stitches placed. returned to work afterwords the same day.

Fall to the walkway or working surface

Employee was up on Loader and was getting ready to get off and tripped over his own foot falling to the ground. Has Bruised or slight cracked rib.

EMPLOYEE WAS WORKING AND RAN TO SHUT OFF CONVEYOR BELT; STEPPED ON A ROCK AND TWISTED RIGHT ANKLE. ANKLE SWELLED AND TURNED BLACK AND BLUE, ALOT OF PAIN.
