While loading the railcar, EE went to raise the access platform so the railcar door could clear the platform. EE grabbed the platform with right hand and pulled up to lift it. EE felt a pop in right elbow. Diagnosed with tennis elbow.

Using a Come Along to pull a piece of metal into place to weld it. EE was jacking the Come Along & a link in the chain on the Come Along failed, springing back and struck EE in the face-above eye and on forehead.

Rigging scale safety rail on the shop floor with telehandler. While doing this, rail shifted, EE tried to move out of the way, but was struck by the rail, falling to the ground, rail landing on EE's knee

EE was walking to a conveyor belt to check the condition of the belt splice, when EE slipped on a patch of ice and fell. Initial diagnosis was shoulder pain, later diagnosed with rotator cuff tear.

Attempting to take sample from discharge chute. EE believes shovel contacted moving belt under chute, causing it to jump. EE's RT pinky finger came into contact with the conveyor frame. The impact caused the nail to catch on the frame and pull nail from nail bed, the nail lacerating the fingertip.

Rocks collected in a chute going to screen deck. EE was removing rock from the chute, using a spud bar. The bar slipped, pinching a finger between the bar and a beam supporting the screen deck. EE sustained a angulated & rotated fracture to the left 5th (pinky) finger, requiring a splint and possibly surgery.

While lowering the new housing tube into position, the employee's left index finger was smashed between the tube and the side sheet of the vibrating screen machine. Finger was lacerated but not fractured. six stitches.

Greasing the loader. Several grease fittings weren't taking grease, needed to be changed. Employee climbed ladder, attempted to open door from too far down. Employee lost grip as they opened the door with the right hand, causing them to lose balance and fall, landing on right hip.

A log got stuck in the dredge hopper. EE got supports to block the grizzly from moving. EE was positioning the support by reaching through the handrails. The support slipped, the grizzly kept descending and struck EE's hardhat. EE pulled head back, cutting nose on the handrail.

Breaking oversized rock with excavator. While placing tool onto rock, the boom pressure remained, causing excavator to flip onto its side. EE had lap belt on, but upper body thrown into cab sides, fx clavicle. EE had to jump off about 8ft, spraining both ankles.

Using a loader to move an oversized rock. As he raised bucket & started forward, rear tires of loader came off ground, causing rock to fall out of bucket. Loader rocked back/forth, causing employee to bounce around, developing back pain, he was wearing seat belt.

Using a pry bar to get a cable unstuck on a loading spout for the south mineral filler bin. The cable slipped, causing pry bar to come off spout, his RT hand came in contact with an angle iron, across the top of 5th finger & knuckle. Fracture to right 5th finger. Splinting. His left hand slid on handrail, his left 4th & 5th fingers split between the handrail support, pulling his 5th finger backwards. He went to occ health & was given a splint. No fracture.

The door on side of Stedmon Mill was frozen. Employee was trying to pry it open. He lost grip and the door sprung back catching tip of his left little finger between the door frame and the door. The door is constructed of 3/4" steel plate. The frame is also steel. Employee received a fracture to his left pinky finger.

EE was operating track mobile to position onto tracks to load railcars. As he was positioning the track mobile to go back on rail, left rear tire rim failed. A piece of the rim swung up against side of the cab, striking cab & employee. The side glass was open and employee leaned out to line up wheels when rim failed causing employee to be struck in the face. Trauma/contusions.

Employee was tightening a large bolt with a ratchet. The ratchet slipped and employee's right hand struck solid steel structure smashing his right pinky finger between the structure and the ratchet. Employee's right (pinky) 5th finger was crushed. Employee was restricted.

Employee was cutting rubber skirt for an 8 X 20 wash screen. Employee was cutting a hole for a spray pipe to go through the skirt. Employee was finishing the inside corner when the knife cut out and contacted employee's left thumb on the first knuckle resulting in a laceration. Three stitches were required.

Reported to his supervisor on 1-23, went to his family doctor on 1-24. Was sent to company clinic on 1-28-13. Diagnosed with pain/sprain to his left knee. Is restricted.

Employee is a front-end loader operator and operates the bucket with 2 fingers on one hand and uses the other hand on the stirring wheel and does this repeatedly all day long for several years. Per employee his hands have been bothering him for a while but has not got any medical treatment until 1-02-13. Employee is restricted, is still working.

Employee was opening a crusher. While adjusting the crusher, the jack fell over and employee reached out to grab it and then the head of the jack retracted catching the employee's right fingertips. He received fractured/crushed right 3 fingers. The right forefinger could not be repaired and the tip was removed. Employee returned to restricted work on 6-23-12.

Employee was walking down small hill on road by the Lab. His feet slipped and went out from under him causing him to fall to the ground and landing on his right side. The hill was snow covered. Employee had a contusion to his right elbow and right ribs. He was restricted but there was work for him within the restrictions.

Employee was repairing the screen deck and was working on a piece of steel to bend it backwards using a sledge hammer. The sledge hammer bounced and employee lost grip and his right hand slipped and struck a metal burr on sheet of steel. Employee received a laceration to his right 5th finger. Stitches were required. Employee was tightening screw bolt with electric impact. A foreign body flew between safety glasses and his left eye. Employee worked the rest of the day. Next day had to go to the company clinic. Employee was diagnosed with conjunctivitis, irritation and laceration in his left eye. Employee was given a prescription. No lost or restricted time.

Employee was doing clean-up. Employee was shoveling spilled material around the plant and felt pain and numbness in his wrists. Employee claims he had felt the pain several days before but did not report it until 9-30-06 because he thought the pain would go away. He sought medical treatment on 10-02-06 and he is restricted.
