On Saturday October 30, at 10:00 am a miner was struck in the head and neck area with a Cat 324E long reach excavator bucket causing an injury to EE's neck. EE was able to return to work with restricted duties on December 15, 2021.
Miner was working on the exhaust system of a 345 Cat Excavator when they lost balance and fell approximately six feet and hit the ground. They landed on their left side and fractured the radius bone near the wrist.
Miner was attempting to re-install an air condition vent cover when fingers came in contact with the fan.
Employee was working on cleaning out the pan of the primary crusher when he lost his footing which caused him to lose his footing. He twisted his foot and ankle as he fell.
EE was breaking large boulders with 325 Cat Hammer. He had pressure on 24"-36" boulder. When boulder broke, the rear of machine lifted off the ground, when it came back down the EE's elbow struck the pull up bar near the door. Went to Dr, had X-rays, said it was bruised. Went back to Dr on 9/13/10 and said it was fractured.
Stepped on a loose rock and twisted ankle.
Pulling dewatering hose over a rock pile while loading a shot, hurt low back & left leg
Employee was under a pickup truck to adjust linkage. Truck was knocked out of gear and ran into him. Cuts to face, dislocated shoulder, small broken bone in right hand.
EE LIT HIS LIGHTER TO SEE LEVEL OF FUEL IN MACHINE TANK. FIREBALL ERUPTED, BURNING EE'S FACE.
Rock flew from passing quarry truck - was stuck between tires - Hit emp. in head knocking him down - Cuts & bruises - possible broken nose - Stitches required at emergency room - dismissed.
TRYING TO PRY OPEN A STUCK DOOR WITH A METAL BAR, IT SLIPPED, THROWING HIM OFF BALANCE. HIS RIGHT FOOT GOT CAUGHT, TWISTING HIS ANKLE.

CRANE WAS LOWERING LARGE PIECE OF STEEL INTO PLACE. EE WAS GUIDING IT, SLIPPED AND CAUGHT LITTLE FINGER BETWEEN STEEL AND POSITION, AMPUTATION OF END OF FINGER BETWEEN 2ND JOINT AND FINGERNAIL .
