EE was cutting water hose with personal pocket knife when knife closed on right index finger.

EE was performing maintenance on a set of screens. While inside the screen deck EE looked up and a foreign object fell between EE's safety glasses and EE's eye. EE washed it out and reported to supervisor. The next Monday (1/13/2020) it was bothering EE. At this time supervisor took EE to clinic.

Employee's had right hand resting on the side of the feed hopper near the top of it. EE was looking down into the crusher to see if the saddles were worn. As the feed hopper was being lifted EE did not remove their hand. This caused their fingers to get caught between the top of the feed hopper and the support beam the feeder slides back and forth on.

Secondary plant chute was brought down to ground level for maintenance on sides. Two temporary legs where attached to lift up. Left leg came unattached causing the chute's right leg to slide off a metal part onto employee's foot. Bottom of temporary leg entered the portion of steel toe boot where steel and boot material meet.
Employee was in the process of cutting blast hole tubes in half to tube the blast holes that EE had drilled. EE's knife slipped while cutting the tube and cut EE's leg above the knee.

Employee was attempting to park the Kawasaki Mule at end of shift. Employee was turning the Mule as the pathway, which was flat, changed from gravel to asphalt and the Mule turned over on its side. Injuries were scraped left forearm, broken finger tips to left ring finger and left middle finger. Left palm required 3 stitches and the left index finger required 6 stitches.

EE was installing a rubber snout on the discharge hopper of C33 to C34. He was using a box knife to trim the rubber when the knife slipped and cut his lower left arm.

Top radiator hose was being removed from A 2468 skid steer, while pushing hose from radiator, right hand slipped from hose striking hose clamp cutting right outer knuckle.
Employee was moving a 20 lb water pump for back-hoe to pass. When employee lifted the pump out of the way, his foot slipped in the mud causing him to twist his lower back.

Customer truck was descending down the access road with 18.77 tons of jetty stone and blew both rear air pods causing a braking system failure. The drive jumped from the truck and the truck ran over the driver's legs.

Rock was lodged inside cone crusher. Employee got inside cone crusher and was twisted around to grab a rock. While pulling at a rock his back made a popping sound. Emploee was treated at local clinc; was diagnosed with a pulled muscle.

Employee was cleaning a dust chute that was clogged. He was poking the buildup with a shovel from the top. When the material gave way, the employee's right wrist came down and hit a piece of angle iron on top of the box. Employee has been instructed to use proper tool in the proper manner for the job.

THE INJURED EE WAS ASSISTING A CO-WORKER IN TRYING TO LOOSEN A TIGHT BOLT ON A 4248 JAW CRUSHER. THEY WERE USING A LONG PUNCH & A HAMMER TO TRY TO DRIVE THE BOLT OUT. THE EE'S HAND SLIPPED JUST AS THE OTHER EE SWUNG THE HAMMER TO HIT THE PUNCH. THE HAMMER STRUCK THE EE'S RT.RING FINGER.

MALE WAS FLEEING FROM POLICE FROM A ROUTINE SAFETY CHECK. HE RAN ACROSS ABC COKE PLANT'S ROAD & CLIMBED OVER A FENCE ONTO OUR PROPERTY. SRM AGGREGATES NOTIFIED POLICE TO ADVISE THEM THAT THERE WAS A MAN HANGING OFF OF OUR HIGHWALL. DETECTIVE CAME TO OUR PROPERTY. THE MALE HAD MOVED & THE OFFICERS BACKED OFF. THE MALE WAS SPOTTED AGAIN. ADVISED THE MALE HAD FALLEN FROM THE HIGHWALL.
