Employee was coming down the steps from the #1 screen. When EE got halfway down the steps a rock fell from the #2 conveyor, which is above the steps. The rock struck EE on the top of the head in the center. This caused a laceration on the top of EE's head, which required staples from the hospitals physician.

Showing all 29The employee was shoveling/cleaning up on the S4 conveyor platform. The employee made a bad step through the floor and while falling hung upper left arm on a sharp piece of metal causing a long cut that required medical treatment including stitches.
A large piece of metal had lodged in the cone crusher North while trying to remove the metal, the crusher turned and smashed the employee's thumb against the bowl.
Employee was cleaning spillage under jaw crusher discharge belt when came in contact with moving belt and pinned employee against belt and conveyor frame, causing injury to shoulder.

Employee was climbing onto D65 Dozer. There was mud on the tracks causing him to slip and fall injuring his shoulder.
The employee was working on an impact crusher internal parts, when removing the main curtain hanger left side came loose the right side broke landing on his hand. There was no way to see the broken side of the shaft holding up the curtain. The right side hanger should have held.
Employee was loosening bolts on an electric motor in order to tighten the V Belts, the ratchet & socket slipped and he mashed his thumb.
The employee was removing a hydraulic line on a hydraulic cylinder, when the pressure in the system hit him in the face hurting his eye. The hydraulic fluid knocked off his safety glasses and hard hat.
While Employee was climbing up the stairs on a volvo haul truck, he lost his footing and fell to the ground.
Employee's were cleaning up under Jaw Crusher, co-worker was throwing shovel full of muck out from under Jaw when his shovel struck the injured EE's hand causing a cut to 2 fingers.

He strained his back loading powder monkey with 50 lb bags of ANFO.

EE WAS WELDING INSIDE A TRANSFER CHUTE WHEN HE LOST HIS FOOTING AND SLID ABOUT 12", STIRKING HIS TESTICLES ON A METAL BRACKET.
A CONTRACTOR WAS CHIPPING A WELD WHEN A PIECE OF SLAG FLEW UNDER HIS SAFETY GLASSES AND HIT HIS EYE. HE DID NOT REPORT THE INJURY THAT DAY. HE MISSED HIS NEXT DAY AND REPORTED FOR WORK ON THE 2ND DAY AFTER THE ACCIDENT. THE EYE WAS CLEANED, NO PERMANENT DAMAGE.
INJURED WAS RUNNING A LOADER CLEANING FACES WHEN THE CUTTING EDGE CAME OFF. HE BENT OVER TO LOAD CUTTING EDGE INTO BUCKET AND PULLED BACK THE CUTTING EDGE WEIGHS ABOUT 65 LBS.
WHILE USING A PRY BAR ON A ROCK THAT WAS JAMMED IN A BELT TRANSFER POINT, THE BAR SLIPPED CAUSING THE EMPLOYEE TO SMASH HIS FINGER AGAINST A ROCK. HE BROKE THE TIP OF HIS LEFT RING FINGER .
MOVING OXY CAGE WITH BACKHOE & HE WAS STEADING THE LOAD WITH HIS HAND HIT A BUMP & THE DOOR CAME CLOSE ON IT
ADDING 40' PIECE OF PIPE ON LAKE BEHIND DREDGE. THE WRENCH THE EE WAS USING SLIPPED ON BOLT BEING TIGHTENED AND CAUSED BOAT TO SHIFT AND EMPLOYEE FELL INTO FLOOR OF BOAT. EMPLOYEE CAUGHT HIMSE LF ON HIS HANDS IN FLOOR OF BOAT.
EE WAS PULLING BEARING HOUSING OFF OF A FINISH SCREEN WHEN IT UNEXPECTEDLY CAME LOOSE AND FELL ON HIS CHEST.
MAN WAS INSTALLING DRAIN PIPE, WHILE TRYING TO LINE PIPE HIS FINGER SLIPPED, INTO THE CONNECTION. PIPE SLIPPED TOGETHER WITH FINGER IN THE CONNECTION.
CARRYING 60 LB I-BEAM THROUGH JAW FEEDER, SURFACE WER, SLIPPED AND FELL.
WHILE TIGHTENING DOWN TENSION ROD ON JAW CRUSHER, ROD BROKE AND HIT HIM IN THE LEFT SHIN. TOOK HIM TO EMERGENCY ROOM; NO IMPAIRMENT, ABLE TO RETURN BACK TO WORK.
REPAIRING P4 CONVEYOR BELT, EE STATES, IS WHEN HE THINKS HE HURT HIS BACK.
