Struck by falling object

Employee was coming down the steps from the #1 screen. When EE got halfway down the steps a rock fell from the #2 conveyor, which is above the steps. The rock struck EE on the top of the head in the center. This caused a laceration on the top of EE's head, which required staples from the hospitals physician.

A large piece of metal had lodged in the cone crusher North while trying to remove the metal, the crusher turned and smashed the employee's thumb against the bowl.

Fall from scaffolds, walkways, platforms

Employee was changing the pre-screen on a machine and slipped of a platform approximately 3 feet off the ground landing on left shoulder. Suspected rotator cuff injury.

Employee was climbing onto D65 Dozer. There was mud on the tracks causing him to slip and fall injuring his shoulder.

HE WAS PRYING ON BENT HANDRAIL ON TRUCK TARP PLATFORM THE PRY BAR SLIPPED OFF AND MASHED HIS FINGER BETWEEN THE BAR AND HANDRAIL AND HAD FOUR STITCHES ON LITTLE FINGER.
