Fall onto or against objects

The employee was shoveling/cleaning up on the S4 conveyor platform. The employee made a bad step through the floor and while falling hung upper left arm on a sharp piece of metal causing a long cut that required medical treatment including stitches.

Employee was cleaning spillage under jaw crusher discharge belt when came in contact with moving belt and pinned employee against belt and conveyor frame, causing injury to shoulder.

driller got out of drill to perform work around drill boom, turned ankle on rock on floor causing him to fall. left work at end of shift and said had no injury on following day went to doctor and found had broken bones in foot

Struck by falling object

The employee was working on an impact crusher internal parts, when removing the main curtain hanger left side came loose the right side broke landing on his hand. There was no way to see the broken side of the shaft holding up the curtain. The right side hanger should have held.

The employee was removing a hydraulic line on a hydraulic cylinder, when the pressure in the system hit him in the face hurting his eye. The hydraulic fluid knocked off his safety glasses and hard hat.

Employee's were cleaning up under Jaw Crusher, co-worker was throwing shovel full of muck out from under Jaw when his shovel struck the injured EE's hand causing a cut to 2 fingers.

Employee was working on replacing a hydraulic cylinder on an underground drill. While driving the pin in the cylinder mount, a piece of the pin hit him on the right leg.

He strained his back loading powder monkey with 50 lb bags of ANFO.
