EE had been checking a light on EE's Dozer, EE was attempting to get off the Dozer when EE grabbed the CB Antenna instead of the hand hold, the antenna slid causing EE to lose EE's balance and falling to the ground, EE fell approximately 7'.

EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.

EE had just finished fueling the grader, as EE was dismounting the grader EE's hand slipped and EE tried to grab back a hold when EE's glove got caught on the corner of a guard, cutting EE's right hand.

A second EE backed his hauler into the hauler injured EE was sitting in waiting to get loaded. Proper safety procedures were not used in this incident. Both men said they were OK & did not need medical attention. On 12-15-15 injured EE came into work and said his back was hurting and wanted to get checked out. EE went to ER & was told to follow up with his Dr.

EE arrived on the job at approx. 5"50 am. EE told the foreman his leg was hurting & he could not run the dozer. The foreman told ee to go to the parking lot & when he got the job going he would be back. When the foreman returned he found ee unconscious. He started CPR & continued it until the rescues squad arrived. EE was pronounced dead at Hospital

The hauler was on a slick incline and could not pull up the hill the hauler driver requested that the dozer give him assistance, the hauler tires were spinning and when the tires got traction, the hauler jerked, causing it to jerk the operator, hurting his back.

Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.

Employee was operating a D10 T Dozer, when smoke started coming into the cab of the Caterpillar Dozer. He turned his key switch off, got his fire ext. & exited the dozer. He used 3 fire ext. trying to put fire out, but could not ext. the fire. After an investigation of the incident it appears to be an electrical fire, that has destroyed the D10T Cat. Dozer.

Heat from a defective take-up roller bearing is believed to have caught the belt on fire causing approximately 2000 ft of belt structure and the #3 head house to be damaged beyond repair.

EE alleged that the bed of the Cat 789C hauler free fell and undestinational distance, causing an injury to his back. EE said he raised the bed of the hauler & then as he was lowering it the bed stopped, he pushed the down lever & the bed of the hauler; FREE FELL down against the frame. The hauler was checked by the mechanic & no problems were found.

Removing radiator cap, antifreeze came out burning employee's hands, he then jumped away from the area landing awkwardly on both feet. He sustained a fracture to his left foot and bruised his right ankle, also received burns to both hands.

Employee was using a sledge hammer to remove a tooth off a loader bucket of a 994. A piece of metal flew off and penatrated the left knee. This caused a lot of bleeding and surgery to remove the imgesef piece of metal.

EE was cleaning raw coal crusher using a walk behind BobCat. EE picked up a 5 gal. bucket of mud and strted backing up a steep grade. The BobCat moved forward the EE slipped and fell. his left foot ended up under the BobCat.

EE was putting chains on a motor grader using a hammer. While striking the chain roll pin, a piece of the hammer broke off and struck the EE in the bottom of the right eyelid. A sliver of metal was removed by a doctor; glue was used to seal hole in right eye-ball.

Employee was helping remove a cab from a loader. Was stepping down a 6 foot ladder, pulled a 3/4" drive impact wrench off of the tire of the loader, hitting himself in the face as he stepped to the ground. Causing a cut to the mouth requiring eight stitches and chipped tooth.
