Employee was operating a CAT-777D Hauler. He stated the bed of the truck came down & hit the frame of the truck hard, causing it to shake him.

Employee slipped off the bumper of the fuel truck and fell on the ground injuring his left shoulder. Worked light duty, now missing time {01-16-2012}

EE was cutting a piece of water hose with a pocket knife and cut the top of his left thumb nail. Did not miss work, cut required stitches.
Employee was putting drive line bolts back in a 980G loader. The rachet slipped off the bolt head, striking the employee on the left side of his nose and corner of his left eye. (stitches)
EE was tightening the tire chains on MG-11 and felt something pul in his right shoulder.
Employee was hooking a chain, slipped on the ice and fell on his left shoulder. Note: Did not know the employee lost time until 2/16/09

This is a non-injury accident. A D-11-R CAT dozer caught on fire and burned longer that 30 minutes. No injury to employee. Fire started between shifts, was found burning by the service man. Fire department was called and they put out the fire.

Employee opened the door to enter the cab of a 980 loader. The door swung open hitting him in the left elbow. He did not seek medical attention until 10-18-07.
EE was driving a pin into his excavator track & hit his little finger on the right hand on the track. Mashed his finger & lost his finger nail. Required stitches.
