Employee was operating a remote control continuous miner. He was struck by a rock. He did not survive the injuries.
On December 12, 2009 at approximately 6:38 am, a pre-dawn, low temperature accident occurred on a haulage road at PE Southern Pike Co. The driver of DT 178 exited the loaded coal truck, resulting in his death.
Two employees were struck by the boom of the pivoting hoist. At this point in the investigation, it appears that the boom or boom hoist ropes came in contact with an adjacent structure as the boom was being lowered. The elevation of the boom changed suddenly as the boom hoist rope or boom lost contact with the structure, striking both individuals.
Employee was traveling #2 Longwall Bleeders and when he did not check in, we sent someone in to look for him. He was found unresponsive with no vital signs. The cause of death is still being investigated.

VICTIM WAS ON A BELT CONVEYOR TO CUT OFF A LOOSE PIECE OF BELT. HE FELL APPROXIMATELY 20 FEET TO THE GROUND BELOW. HE DIED ON JANUARY 11, 2011. HARD FROST THE MORNING OF THE ACCIDENT. FAILED TO WEAR COMPANY SUPPLIED SAFETY HARNESS.
Truck traveling from mine site. The victim had truck in high gear, truck was out of control, he put truck in ditch and jumped out. Victim received fatal injury to head.
The investigation is not complete, but as of now it appears that the haul truck he was driving made contact with the safety berm and tipped on its side. The caue of the accident is unknown at this time.
A piece of grating had been moved to insert a piece of pipe. Employee stepped in hole in grating and fell 38' down the shaft. Injury was fatal.
The victim was attempting to deliver an overnite package. She got out of her truck, began walking toward the loader as the operator was backing in her direction to take delivery at the truck as usual. She had never left the truck before. She walked in the blind side(left rear)of the loader and was not seen. She was struck & run over by both left side tires causing fatal injuries.
Employee had just gotten off work, he was driving out of the mine on the haul road and was trying to pass two other employees and got off the road and hit an old gate post.
The individual was struck by a rib roll measuring approximately 20' long X 2' thick X 4 1/2' wide causing crushing injuries.
Employee was driving a loaded hauler when he backed the hauler into a pond with no hesitation.
The victim was operating a walk behind block saw on a ledge. The victim and the saw went over the 9-foot ledge and the saw fell on him. This mine closed in 2005 and the operator did not notify MSHA that mining operations had resumed.
Employee was in the process of setting or had set a roof jack, on the left inby corner or #60 Crosscut between the No 2 and No. 1 entry in the 4 west miner section. A piece of stone fell out of the roof and struck the employee, pinning him to the ground.
The investigation is not complete, but as of now it appears that the employee had washed the #2 drill and was rolling up the hose when a portion of the mine roof collapsed, striking and covering him with a section of the salt roof.
Employee was looking for hydraulic leak and was struck in head by eject all plate. Employee received a massive head trauma and later became a fatality.
Supply car was bringing concrete block into break to build permanent stoppings when chain broke letting supply car roll through curtain crushing foreman between supply car and 94L bridge causing fatal injury.
Surface Mine Engineering Sampler was backing up his flatbed pickup on a a drill pattern. A Drill Operator had dismounted his drill and was walking on the pattern. The drill operator walked behind the pickup and the sampler was not aware the the driller was there. The driller stopped walking and was run over by the flatbed pickup, resulting in fatal injuries.

Employee followed none of the safety & company policies. Allowed his girlfriend on the property and up in the cab of the loader. Also, evidence is pointing to drugs or alcohol. Awaiting the toxicology report.
Employee was trying to untangle cable while it was still on and rotating. Arm was then caught in spool of winch and torn off body which caused his death.
Employee was pinned between concrete structure and wall of hole.
Employee fell approximately 100 feet while dismantling a suspended Safway Systems scaffold. The accident is still under investigation.
Employee was connecting power cord that feeds the dredge.
Employee was standing on the left side catwalk of a 777F rock truck, near the rear of the cab, when the dump bed came down, crushing him between the bed and the catwalk handrail.
Employee got out of cab and stepped toward window & knee gave out. Employee did not slip or trip, footing was good, maintained 3 point contact, from a flat surface, but as employee shifted his weight his knee gave out.
A dragline oiler had been walking the 1570 dragline. He was last seen near the end of a walkway and was later found between the shoe and the revolving frame. There were no witnesses to the event. The employee was fatally injured.
Installing a rolling steel door, the victim was on a stepladder - Lost his balance and fell approximately 7 feet landing on a concrete floor. He died from his injuries. He was taken by helicopter to the hospital (St. Mary's in Evansville, IN.)
Employee was walking at the plant facilities, a cornerstone of the crane collapsed, hitting the employee directly to the body and falling down. Employee was wearing safety equipment as required (capacete). Received multiple injuries to the upper and lower body.
A truck driver was delivering a load of lumber to the mine site. While the fork truck operator was unloading the truck, the truck driver walked around the other side of the truck when the fork truck operator accidentally pushed over a bundle of lumber, crushing the truck driver fatally.
Truck out of control, driver not wearing seatbelt, ejected & crushed.

Employee entered area of accident by foot deviating from procedure. Was found later in bottom of hopper engulfed by stone.
The victim was operating a skid steer loader underneath a belt conveyor that was being dismantled. A co-worker was in an elevated manlift & was using a cutting torch to remove a piece of 4" metal tubing attached to the leg supports of the belt conveyor. The tubing fell to the ground, tipped over into the front of the loader & struck the victim. He was hospitalized & died on 1/9/09.
