WHILE CUTTING STRIP OF SHEET AT 1/2" STYROFOAM, THE KNIFE SLIPPED AND CUT TIP OF LEFT LITTLE FINGER. CUT WAS EXTENSIVE ENOUGH TO REQUIRE 5 STITCHES.
TRIPPED ON ROCK WHILE CARRYING ROCK AND FELL TO FLOOR. FELT PAIN IN LOWER BACK.
WAS PROCEEDING TO REMOVE STONE FROM ROCK SPLITTING MACHINE WHEN INCIDENT OCCURED. LOWER BACK WENT OUT, COULDN'T LIFT AT THAT MOMENT. VERY SHARP PAIN IN LOWER BACK, ALSO PAIN SHOOTING DOWN LEGS
WAS WALKING IN QUARRY WORK AREA WHEN SLIPPED AND FELL AGAINST EDGE OF QUARRIED BLOCK.
LOADING STONE MANUALLY ON PALLET WHEN FINGER WAS PINCHED BETWEEN 2 STONES. SUFFERED LACERATION ON LEFT RING FINGER.

THE EE WAS DRILLING A LINE OF 1" HOLES ON ROCK SHELF WHEN HE OBSERRED A BUMP ON HIS WRIST.
EMPLOYEE WAS PICKING UP A PICE OF STONE 4 1/2" X 5 1/2" IN CROSS SECTION AND 46" LONG, WHICH WAS LEANING AGAINST THE WALL. HE GRABBED THE TOP PORTION OF THE STONE WITH BOTH HANDS, TILTED IT T OWARDS HIM AND STARTED TO LIFT WHEN THE TOP 12" OF THE STONE BROKE OFF IN HIS HANDS. THE BOTTOM 36" FELL TOWARDS HIM AND LANDED ON HIS LEFT FOOT.
