On 12/02/2020, miner noticed a bulge in right groin area. It was not causing pain or discomfort at the time. On 12/09/2020 during annual health exam, MD confirmed right inguinal hernia. Dr. stated it was not emergency and with COVID restrictions could wait for repair. Workers Comp Dr. exam was on 06/09/2021 and was referred for surgical repair, which was done on 09/30/2021.

On 1-13-12 at approx 8:30 am miner reported that he noticed a bulge at his navel the night before. He stated it was not causing him any pain and was not interfering with his work bagging and stacking 50# bags of rockdust. Dr stated "was most likely a preexisting cond. worsened by lifting".

Employee was attempting to clean chute above rotating feeder while feeder was still running. Little finger on right hand came in contact with moving parts of feeder.

Employee was cleaning out the oscillator with a t-handled trenching shovel while the oscillator was running. The shovel caught on a bolt on the oscillator pan, forcing the shovel back and mashing his left hand against the hopper.
Employee was helping a contractor load and move out their welding machine on the contractor's forklift.The welder was not centered very well on the forks, and when it was picked up approx. 15" it started to fall. He stepped forward to help steady the welder and at the same time the forklift operator lowered the forks, setting the fork on his foot.
While grinding a piece of pipe the grinder got loose and ground into his right forearm and knicked his chin.
