Employee stated they lifted something and pulled their back.

Piece of metal flew off of wedge and hit upper lip and cut.

Employee was walking backward with jackhammer in hand when he tripped and fell.

Piece of rock fell on top of right foot.

Operating jackhammer and rock fragment flew up and hit eye.

Hose blew off jackhammer and hit in mouth.

Putting hold in with jackhammer and fell against jackhammer handle. Bruised ribs right side.

Hooking up tool building in bole. His finger was mashed between chain & dogs.

Employee was hitting wedge with sledgehammer to split granite and wedge broke hitting employee in the mouth. 2 teeth were cracked.

Hooking bob cat and slipped and hit side (left).

Moving piece of granite and cut finger. Middle finger left hand on granite.

Opening powder magazine and door slipped from hand and mashed fingers on his hand. Cut tip of thumb almost off, doctors reattached.

Fell on sharp rock & hit right knee.
