On 7/23/19 an employee was dismounting a haul truck and twisted their L-knee. Initial X-ray and medical evaluation showed negative findings. An MRI on 9/12/19 showed a hairline fracture and local management later learned on 9/16/19 that employee had been receiving PT since August.

Scaler working a loose brow was using maximum pressure on the pick when slab broke loose. Slab hit the boom of scaler, lifted the rear of machine appx 5 feet off the ground. Scaler slammed back to the ground jarring the operator.
Blaster was pulling the loading hose out of the hole the ANFO pot went empty causing the compressed air in the pot to blast out the hose. Blaster could not hold onto the hose and turned to get away. When EE did EE slipped and fell hitting EE's left knee on a rock as EE went down which dislocated EE's kneecap

A roof fall occurred in the underground magazine area between 7/29, Thursday, AM and 7/30, Friday, AM. It was discovered during the morning mine inspection on 7/30. A slab of rock approximately 3 inches thick and measuring roughly 10 ft x 10 ft fell from the roof onto a dry van used to store ANFO breaching the roof.

Employee was working in a loader for entire 10 hour shift. Next day at beginning of shift, employee reported that his eyes were stinging and red. Took employee to Promptcare and reported that eyes had an infection in both eyes due to diesel smoke. Employee was given antibotics and steroids for treatment. No work was missed or restriction of duty.

While working at the underground quarry a piece of rock fell from the ceiling striking & cutting left hand.
Loading ANFO into the bulk truck, cut hand (needed stiches) on the handle on the ANFO bin.

EMPLOYEES BEGAN SCALING THE LOOSE MATERIAL FROM THE FACE WITH PRY BARS, ONE EMPLOYEE FINISHED AHEAD OF THE OTHER AND PLACED HIS HANDS ON THE HANDRAIL OF THE WORK BASKET. AS THE EMPLOYEE PLACED HIS PRY BAR IN POSITION, A LARGE MASS OF ROCKS FELL OUT OF THE FACE TOWARDS THE WORK BASKET AND LANDED ON THE OTHER EE'S LEFT HAND, WHICH WAS ON THE HANDRAIL. TORE END OF LITTLE FINGER OFF.
