Employee was cleaning rock dust bin #2 from inside of bin #2; material on sides of bin #2 collapsed, engulfing employee; causing his death.
Just removed crusher base under which was a small rectangular opening through which power cable had run. Hole not noticed; blended in with expanded metal grating. Cleaning up work in preparation to barricade the crusher opening/area. Stepped into hole, slightly larger than a large boot footprint, leg dropped through, straining the knee of the other (left) leg and right shoulder.
EE was hooking up welding trailer, he squatted down to hook up safety chains and when he stood back up his left knee felt like it came out of the socket. EE went to ER and doctor told him his knee was fine and to RTW as scheduled. EE went to Dr approx 4 weeks later and set up for surgery. He had previously had surgery on knee from ATV accident when worked at Grassy Store.
Employee was clearing loose gravel from area of the tailgate; tailgate suddenly closed, mashing ring finger of left hand.
Employee was shoveling belt line when he felt something pop in his back.
OPERATOR WAS TRAMMING DOZER ACROSS FILL AREA WHEN HE ACCIDENTALLY ENGAGED PARKING BRAKE. THIS CAUSED EMPLOYEE TO STRIKE HIS HEAD AGAINST REAR VIEW MIRROR FRAME, CAUSING SCALP LACERATIONS.
TAKING HOSE LOOSE ON 777 THE O-RING BLEW OUT SPRAYING HYDRAULIC OIL ON HIM.
COMING DOWN STAIRS TO CATWALK FROM THE TOP TO THE PLATFORM. BOOT HEEL CAUGHT END OF STEPS, KNEE POPPED. THERE WAS NO PAIN AT THAT TIME. BECAME A LITTLE STIFF AND STARTED HURTING THE NEXT DAY.
WALKING DOWN FROM 5 1/2' CONE TO THE WASH. SLIPPED AND TRIED TO JUMP FOREWARD SO I WOULDN'T FALL, AND HIT MY HEAD ON A STEEL BEAM SUPPORT.
EMPLOYEE RAN OVER ROCKS JARRING HIS BACK & ABOUT 10:00 PM RAN OVER A BOULDER & DROPPED OFF THE BOULDER SEVERELY JARRING HIS BACK. EMPLOYEE WAS IN PAIN ALL NIGHT WITH BURNING SENSATION IN RIGHT LEG & FOOT. THE NEXT MORNING HE WAS UNABLE TO WALK OR MOVE WELL.
