EE was adding hydraulic oil to an outby bolter, as EE lifted the five (5) gallon can of oil EE felt pain/strain in lower stomach (groin area hernia). EE continued working without restrictions performing normal occupation until EE had surgery date of 2-19-19.

EE was servicing the batteries for the ram car. As EE bent down to lift a gallon jug of water, EE twisted causing pain to mid/lower back. EE's doctor gave EE a full release without restrictions until 11-20-18, at which time EE's doctor placed EE on restrictions.

EE was tightening the cats on a continuous miner (CM). As they were positioned on the mine floor beside the CM, a piece of loose draw rock that was resting on the top of the CM shifted and fell striking them on the right ankle/foot, causing a fracture. They saw the loose material prior to squatting down, but did not think that it would move. EE began missing time on 10-29-18.

EE was washing and servicing the continuous miner (CM), as ee was removing a rock from on top of the CM, pinching left hand between the rock and the top of the CM. EE suffered a fracture to index finger and was given a full release and ee did not miss work as a result of this injury. EE returned to work on 10-23-18 performing normal occupation without restrictions.

A fall of the roof occurred in the 3rd NE Mains, 4th North Intake, crosscut 46, from #3 entry toward #2 entry. The fall measured approximately 50ft. in length, by 20ft. in width, by 7-15ft. in height. This area has had additional standing support installed and the primary escape way has been re-routed.

EE was observing mechanics repair the mesh handling tray jacks on a walk-through bolter. As the mechanics re-positioned the tray, the safety chain broke causing the tray to strike EE on the right shoulder & upper back. EE suffered 2 fractured ribs and ee began missing time on 09/17/2018.

EE was operating a walk-through bolter installing rib bolts, as ee attempted to place a plate on the bolt, ee dropped the plate onto forearm causing a laceration. EE received 2 sutures, ee did not miss time and returned to work on next scheduled shift performing normally scheduled occupation.

Employee was exiting a diesel mega trac and twisted their left knee. Employee continued to work without restrictions performing normal occupation until surgery on 10-30-18. Employee began missing on 10-31-18, they worked the day of their surgery as well.

Employee was loading wire mesh onto the mesh handling tray. As they lifted the mesh they felt pain in their right shoulder, they continued to work without restrictions until placed off work by a doctor on 11-5-18.

A fall of the roof occurred in the 2nd SE Mains (3rd54 Supply Road), #5 entry, crosscut #36, intersection. The fall measured approximately 20ft. in length, 19ft. in length, & 7-15ft. in height. A fall rehabilitation letter has been submitted to the local MSHA office.

EE was operating a walk-through bolter installing roof bolts. While EE was pushing the bolting tray off of the bolter, EE felt pain in the lower back, and transported to a clinic x-rays were conducted with nothing conclusive. Being treated for a strain and placed on restrictions that prevented EE from performing normal occupation. EE began missing time on 08-20-18.

A fall of the roof occurred on the 1st NE Mains (1st54 Supply Road), #5 entry, crosscut #36, intersection. The fall was approximately 20ft. length, 20ft. width, & 7-15ft in thickness. A fall loading plan has been submitted to the local MSHA District 10 Office.

EE was stacking/storing wire mesh onto the walk-through bolters mesh handling slide. As EE was handling the mesh, the tip of EE's finger caught on the mesh causing a laceration to the tip of the finger. EE received 4 sutures to the tip of the finger. EE received a full release to perform normal occupation. EE did not miss time as a result of this injury.

EE was operating a walk-through bolter installing roof bolts into the mine roof. As EE was pushing a bolt into the drilled hole EE felt pain in the right shoulder. EE continued working without restrictions until EE was placed off work on 7-25-18 for a strain.

EE was cleaning up around the 4C Header. EE lost footing and fell to the mine floor, ee felt pain in right shoulder/ankle. EE was treated for a mild strain, given a full release to regular occupation. On 7-16-18 ee seen doctor and was placed on restrictions that prevented ee from performing normal occupation. EE began missing time on 7-17-18.

EE was operating a walk-through bolter installing roof bolts. While ee was pushing the bolt into the mine roof, ee felt pain in lower back. X-rays were conducted with nothing conclusive. EE is being treated for a strain and placed on restrictions that prevented from performing normal occupation. EE began missing time on 06/14/2018.

EE was removing the collar off of the mesh tray jack, a piece of draw rock came from the rib/roof striking EE in the head & right shoulder. The force of the rock drove EE's head and left side of EE's face against the dust box. EE suffered fractures to the left side of EE's face, below EE's eye. EE began missing time on 6-11-18.

EE was operating a walk-through bolter installing rib bolts. While EE was installing the bolt, EE applied boom pressure/rotation and EE's glove caught on the bolt causing left hand to twist. X-rays revealed fractures to the middle & ring fingers. EE has been placed on restrictions that prevented EE from performing normal occupation. EE began missing time on 06-05-18.

EE was operating a walk-through bolter installing roof bolts. While EE was removing drill steels from the mine roof, EE felt pain in EE's right shoulder. X-rays were conducted with nothing conclusive. EE is being treated for a strain and placed on restrictions that prevented EE from performing EE's normal occupation. EE began missing time on 06-01-18.

EE was operating a diesel getman on the supply road. EE encountered a loose concrete block on the blind side, which caused EE to be bounced in the seat. As EE bounced, they felt pain in their back. EE was given a full release to return to work without restrictions. On 05-08-18, EE had an appointment and the MRI revealed two herniated discs, EE was placed off work at this time.

A fall of the roof occurred on the 2nd SE Mains, #1 Unit, crosscut 148, entry #2 (intersection). The fall measured approximately 20' length, 18'-20' width, & 7'-15' thick. The fall has been cleaned up according to our approved rehab plan, additional support has been installed in the area, with the fall area being arched.

EE was a passenger on a 10-man mantrip. The driver encountered uneven bottom which caused EE to be bounced in EE's seat. As EE bounced, EE struck EE's head against the canopy of the mantrip, causing pain to EE's neck. EE continued working without restrictions until EE was taken off work on 04-04-18, at this time EE began missing time.

A fall of the roof occurred on the 1st NE Mains (1st 54) in the #5 entry, the pillar line fell between crosscuts 26-27. The fall measured approximately 65' in length, by 18'-20' in width, & 7'-14' in height. The fall was in a secondary escapeway entry, the lifeline was re-routed, flagged, mapped, and all persons were notified of the change.

EE was operating a walk-through bolter and was drilling a hole into the mine roof in preparation for installing a roof bolt. During this process, a piece of draw rock slid off of the wire mesh striking EE in the right hand. EE was transported to ER where EE had surgery and is expected to make a full recovery and return to normal occupation within 5-6 weeks.

EE was checking supplies on the walk-through bolter and as ee exited off of the bolter, ee slipped and fell to the mine floor, striking right shoulder against the floor. EE continued working without restrictions performing normal occupation until 03-05-2018. EE began missing time on 03-05-2018.

As employee was getting out of the scoop operating compartment, they felt pain in the left elbow/arm. Employee began missing time on 01/19/2018.

Employee was walking on the surface of an underground mine to gather materials for EE's shift. As EE was walking, EE slipped on uneven surface that was slick from snow causing EE to fall to the ground. As EE fell EE caused a fracture to EE's left leg/ankle. EE began missing time on 01-16-18.

As ee was stepping up onto bolting platform, ee lost footing causing ee to scrape right shin on the frame of the step. EE continued working without restrictions performing normal occupation. On 01/17/18 ee was referred to physical therapy and placed on restrictions that prevented ee from performing normal occupation, ee began missing time on 01/17/18.

Employee was picking up a bucket of belt chains, as ee lifted ee strained right side of body, rib area. EE began missing time on 1-9-2018.

Employee was installing a roof bolt by operating a walk-through bolter. As ee was pushing the bolt into the drilled hole, ee felt pain in left shoulder. EE was placed on restrictions on 1-9-18 that restricted from performing normal occupation. EE began missing time on 1-9-18.

A fall of the roof occurred on the 1st NE Mains (1st54), #4 entry (belt entry), crosscut #11, intersection. The fall measured approximately 20' length by 16' width by 12'-16' height. Additional support has been installed in the area with steel arches through the intersection.

EE was operating a walk-through bolter, installing roof bolts in the #2L face. During the process of drilling the hole, the drill steel became stopped up with dust and EE did not let off of the boom/rotation pressure quickly enough, resulting in the steel bending and striking EE in the left side of the face. EE began missing time on 11-21-17.

Employee was unloading 42" crib block from a supply trailer. While pushing the block away from them toward another employee, they felt pain/pop in the right bicep. Employee continued to work conducting normal occupation without restrictions. On 12-12-17 employee was placed on restrictions that prevented them from performing normal occupation.

Employee was operating a shuttle car, while tramming they encountered uneven bottom floor, bouncing them in the seat and causing pain to their neck. EE continued working without restrictions performing normal occupation until taken off work on 03/20/2019.

EE was operating a walk-through bolter installing rib bolts. While EE was pushing a roof bolt into a drilled hole, EE felt pain in right shoulder. EE continued working without restrictions performing normal occupation. On 11-08-17 EE was referred to physical therapy and placed on restrictions that prevented EE from normal occupation. Began missing time on 11-08-17.

EE was riding as the passenger on a golf cart. EE's left foot was under the right leg and when EE's toe caught on a piece of loose coal, the left foot pulled back, causing a strain. EE received a full release and continued working performing normal occupation until the surgery date of 01/03/2019.

Operating a walk-through bolter installing roof bolts at the face, EE was pushing a roof bolt into the drilled hole, the plate slid down and struck EE on right forearm causing a contusion. EE continued to work performing normal occupation without restrictions until surgery was conducted to repair nerve damage to EE's forearm. EE began missing time on 10-03-2017.

A fall of the roof occurred on the 2nd NE Mains (3rd North 54), #4 entry, crosscut 7, intersection. The fall measured approximately 20' length by 20' width by 7'-12' thickness.

EE was setting bits on a continuous miner, as EE struck the bit block with a hammer, EE inadvertently stuck right ring finger, causing a laceration, EE received sutures as a result of this injury. EE did not miss work for this injury.

A fall of the roof occurred on the #4 unit, 1st SW Sub-Mains, #0 entry, crosscut 18, intersection. The fall measured approximately 20' length, 20' width, 8'-14' height.

EE was moving a chunk of coal out of the walkway beside the right sub, as ee lifted the chunk of coal, ee pinched finger between the chunk of coal and another loose rock on the mine floor. EE received sutures as a result of laceration. EE did not miss time as a result of this injury.

EE was picking up a block & stacking a brattice wall, as EE lifted and twisted EE felt pain in the lower back. EE continued to work without restrictions until 1-21-19, at this time EE was placed on restrictions that prevented EE from performing EE's normal occupation.

EE was walking underground and stepped on uneven mine floor twisting right knee. EE worked normal occupation without restrictions until surgery date on 9-5-17. EE will begin missing time 9-6-17.

Employee was operating a walk-through bolter, once EE completed the outside bolt, EE swung in and EE left the canopy too close to the mine roof. As EE swung in, the canopy caught on a previously installed bolt/plate causing the canopy post to disconnect from the frame. The corner of the canopy struck EE's face, directly below the eye, causing a small laceration which was glued.

Employee needed to extend the ventilation curtain and as employee exited the back of the bolter by stepping down approximately 1 foot to the uneven mine floor, employee twisted the left ankle. Employee began missing time on 5-1-17.

A fall of the roof occurred in the 1st NE Parallels, #00 entry, crosscut 19, intersection. The fall measured approximately 20' length, by 20' width, by 7'-12' height. The area will be sealed off by the construction of A-Seals.

Employee was in the process of removing a continuous miner bit from the bit sleeve. As EE struck the bit with a hammer, EE struck index finger/hand in the process causing a laceration to finger. EE received 4 sutures as a result of this injury.

Employee was in the process of installing a roof bolt, as ee adjusted the boom on the bolter, a roof bolt from the bolting tray came in contact with a hose on the boom, the roof bolt struck the employee in the mouth causing a laceration, ee received six sutures.

Employee was operating a continuous miner and turning a crosscut. As employee was sumping the continuous miner into the coal, a piece of loose draw rock mixed within coal spun off of the tail of the continuous miner striking employee in the lower left leg. Employee began missing time on 3-17-17.

Employee was removing a bit from the continuous miner head. As employee was striking the bit with a hammer a piece of metal broke off striking employee in the hand. Employee had the piece of metal removed and received 1 suture from the injury.

A unplanned fall of the roof occurred in the 5th South West Panel, crosscut 38 in the #00 entry, which is worked out area (old #4 unit). The fall measured approximately 15'-19' width, 7'-12' height, & 20'-30' in length.

The continuous miner "CM" on the unit was down and in need of mechanical repairs and the CM operator trammed outby the last open in preparations for the mechanic. EE was driving EE's maintenance ride to the CM to make these repairs and EE pulled through a check curtain striking the CM with EE's ride. EE began missing time on 2/7/17 for a bruised/sprained ankle.

In preparations of making a splice the employee was using a belt knife to cut a piece of 42" belt. As ee was cutting the belt ee began cutting toward self, in doing so ee cut right knee causing a laceration and receiving sutures for injury.

EE was walking in the last open crosscut from the #6 entry toward the #7 entry and ee stepped on uneven mine floor, which caused ee to twist right knee. EE continued to work without restrictions until EE was placed off work on 02/03/2017 by ee doctor. EE began missing time on 02/03/2017.

EE was walking across a field to EE's drill rig when EE stepped on the edge of a frozen rut and EE's feet slipped and down onto the rut and twisted EE's right knee. EE returned to work the next day. EE started missing work for surgery on 3/24/17.

EE was assisting with pulling the 1st 54 belt back together for preparations of making a splice. As EE was helping with this process, EE felt a pop and pain in EE's right knee. EE continued to work performing EE's normal occupation until EE's surgery on 3-2-17. EE began missing time on 3-2-17.

Employee was assisting the continuous miner with moving to its next cut, stepped on uneven mine floor, twisting right knee. Employee continued to work without restrictions. Began missing time on 12-19-2016.

EE was removing metal bands from a new set of bundled roof bolts, as EE was attempting to remove the band by pulling on it, the band struck EE on the side of the head, directly above the right ear, causing a laceration. EE received 4 sutures for this injury.

EE was installing a concrete brattice wall and during this process, a piece of loose rib rolled striking EE on the left leg/ankle, pushing EE into the partially built brattice. EE's injury resulted in a fracture to the fibula (lower leg - small bone) and EE began missing time on 11-16-16.

A fall of the roof occurred on the #4 unit (5th SW Panel), inby the unit loading point, #4 entry-face, crosscut 39 (intersection. The fall measured approximately 20' length, 15' width, & 8'-12' height.

Employee was using a wash-down hose to clean the area around the continuous miner "CM" stab jack, which is located beneath the CM tail. As EE was positioning self to better utilize the hose, they struck right ear on a piece of loose rock that was resting on the tail of the CM. Employee began missing time on 10/19/2016.

Employee was handling materials to install a pump, as EE was dragging these materials through mud & water, EE felt pain in EE's lower back. EE began missing time on 09-15-16.

A fall of the roof occurred on the 1st N.E. Sub-Mains, 2A Return, #9 entry, crosscut #10 (intersection). The fall measured approximately 50' length, 15'-17' width, & 18'-19' height.

A fall of the mine roof has occurred on the 1st NE Mains, 1st54 supply road (secondary escapeway), #5 entry, crosscut 21-22. Measuring approximately 30' length, 15'-17' width, 8'-16' height.

Employee was using a belt-knife to cut a piece of mining belt, as EE was pulling on the belt-knife, the belt slipped and EE cut EE's right hip with the belt-knife. EE received sutures as a result of this injury.

Employee was removing the battery plug from a set of charged batteries. As employee was pulling the battery plug out of the batteries, a spark from inside the plug caused burns to right hand. Employee began missing time on 07/22/2016.

Employee was using a rake to clean out from under a unit belt header. When the handle of the rake came into contact with the belt, the handle struck employee above the left eye and employee received sutures as a result of this injury.

Employee was operating a continuous miner and while preforming a face cut, a loose rib struck the left knee. Employee continued working without restrictions until taken off work pending surgery. Employee began missing time on 08/08/2016.

Employee was operating a coal transporter and a piece of material came into the operator's deck, striking EE on the right hand. EE continued to work without restriction until EE began missing time on 6-3-16.

Employee was installing a brattice wall and as employee was handling a concrete block, felt pain in left shoulder. Began missing time on 6-7-16.

As action to terminate citation 9049399, submitting an alleged incident in which the employee refused to fill out an initial injury report. EE was operating a dozer, inhaled fumes and/or fire suppression that has caused respiratory issues months after the alleged incident.

Employee was using an extension pole to unhang the walk-through bolter's cable rom the cable hooks attached to the mine roof. During the process, EE lost their footing on uneven mine floor, causing EE trip and stain left shoulder. EE began missing time on 5-2-16.

Employee was operating a walk-through bolter, as employee was drilling the hole to install the roof bolt, the roof bolt came into contact with the drilling pot, causing the bolt to strike the employee in the face, causing a laceration.

Employee was attempting to remove the pull cable from the sheave wheel. When EE touched the cable the belt shifted, causing a laceration to EE's finger.

A fall of the roof occurred on the 1st N.E. Mains, 1st 54 #5 entry (supply road - secondary escapeway), crosscut 23 (intersection). Fall measured approximately 16'-18' width, by 35'-45' length, by 8'-14' height.

A fall of the roof occurred on the 1st N.W. Mains, 24A Belt Line, crosscut 52, #4 entry. Fall measured approximately 16' width by 20' length by 8'-12' height.

Employee was walking between the rectifier and the unit power sub, EE lost footing on the uneven mine floor, striking EE's shoulder against the rectifier. EE began missing time on 12-4-15.

Employee was handling bolting supplies, as ee attempted to sort the supplies ee elbow came in contact with the bolting plates, causing a laceration to elbow, requiring sutures.

Employee was loading the walk-through bolter with supplies, arm came in contact with a bolting plate, causing a laceration to elbow that required sutures.

Employee was stepping off of a dozer, he lost his footing, slipped and struck his leg on the frame of dozer.

Employee was operating a walk-through bolter in the #5 entry, a piece of loose roof fell striking him on the left hand, causing swelling and pain. He did not miss work as a result of this injury, however, he was required to wear a splint.

Employee was operating a walk-through bolter and the drilling pot suction stopped working. He stepped off of the bolting platform toward the drilling pot to exam/repair the suction hose. A piece of roof fell striking the ATRS and cantilevered in toward the drilling pot, striking him in the hip pushing him toward the mine floor. He began missing time on 9-29-15.

Action to terminate citation 9049092 for what Dotiki views as non-reportable/non-work related: Allegedly employee was removing a check plug from a continuous mining machine drum, EE felt pain in shoulder.

Unstable conditions of a refuse pile, causing unstable belt structure.

Employee was operating a continuous miner (CM) and he was loading the coal transporter, a piece of loose roof fell and landed on top of the CM. He trammed the CM out from unsupported roof and using a 5' pry bar, he attempted to dislodge the material. The material shifted and slid off the side of the CM striking his leg. He began missing time on 9-17-2015.

Employee was pulling a belt roller out of the way, he pinched his finger as he was handling materials.

Employee was operating a continuous miner and had completed the first lift and was setting over to start the second lift. As he was tramming the continuous miner out of the lift, a piece of loose roof fell from between the mesh and the rib, striking him in the back. He began missing time on 8-24-15.

Employee was walking toward the walk-through bolter. EE slipped and fell striking head on the bolter frame.

A fall of the roof occurred on the 2nd N.E. Mains, 3rd North Belt Line,#4 entry, crosscut #4. Fall measured approximately 16' width by 20' length by 8'-12' height.

Employee was handling bolting supplies, as he was walking with the supplies in hand, he lost his footing, falling to the floor of the bolter, causing a laceration to his left forearm. At the request of MSHA, modified to 2 days before returned to work.

Employee was drilling a hole into the mine roof, a loose piece of rock fell striking him on the lower back, causing a laceration.

Employee was wet dusting a punch cut between #6 & #7 entries. His scoop was parked on the inby corner of the #7 intersection, he attempted to reenter the scoop on the rib side, a piece of loose rib rolled off the corner, striking his back and pushing his body into the scoop.

Employee bent over to handle the continuous miner cable, as EE lifted the cable EE felt pain in EE's right shoulder. EE continued to work without restrictions until EE's doctor visit on 12-29-16, at which time EE was placed on restrictions starting EE's next scheduled shift on 01-03-17. EE began missing time on 01-03-17.

Employee was operating a walk-through bolter and install a roof bolt, a piece of rock fell from the rib striking him on the leg.

Employee reported an alleged head injury that was causing him pain to his head/neck. He was seen at a medical facility, given a return to work with no restrictions. However, he was prescribed medication with known side effects that could have rendered him a potential hazard to himself and those who work around him. The MRI results confirmed regular duties without restriction.

Employee was installing a jack pipe, lost hold of the pipe pinching left thumb. Employee received a laceration that required 4 sutures.

Employee was installing a conveyor chain on an outby continuous mining machine, while striking the chain with a hammer, a small piece of foreign object struck employee in the stomach, causing a laceration that required 2 sutures.

Employee was reaching to grab a pin for the bolter when he reached for the pin he cut himself on the metal band that holds the pins together.

A contract employee was cutting wire mesh and a piece of mesh struck the employee on the arm.

Employee was handling bolting supplies, he pinched his finger between the supplies and the frame of the bolter. At the request of MSHA, modified to 5 days before returned to work.

Employee was handling wire mesh, EE felt pain in finger from handling the materials. EE received a laceration that required 2 sutures.

Employee was installing a roof bolt into the mine roof, he felt pain in his shoulder as he was pushing the bolt into the mine roof. He continued to work without restrictions, he was taken off work from the operator pending an MRI (revealed non-conclusive). At the request of MSHA, modified to 17 days before returned to work.

Employee was installing a concrete brattice and as he was aligning the brattice block, he smashed his left hand/fingers between blocks. He started missing time on 3-2-15.

Employee was installing a driveshaft on a battery scoop. He was assisted by another employee to properly align the shaft. In the process of aligning, his pant leg came in contact with a rotating bolt, which pulled his leg toward the bolt and caused a laceration to his left leg.

Employee was installing a brattice and a piece of loose roof fell between the wire mesh and the rib, striking him on the head. He received sutures for the laceration.

A fall of the roof occurred on the 3rd N.W. Panel, crosscut 30, #5 entry, intersection, LOC. Fall measured approximately 15' width by 20' length by 8'-12' height.

Employee went to push a scoop tire over on a trailer to boom it, when bent over felt pain in lower back.

Action to terminate MSHA citation 9041914 by submitting the following 7000-1 under duress. A fall of the roof occurred for what we view as a non-reportable roof fall in the 2nd N.E. Mains x-cut 4 #3 entry, measuring 30' length, by 16' width, by 8'-12' height.

Employee was building cribs on the backside of the belt, workers were handing cribs under the belt. Employee was picking them up and handing them to another worker when the crib was placed on top and pushed back it fell back down onto employee's hand.

Employee was installing a concrete brattice, he slipped and fell striking his right side against the pallet of block. He started missing time on 11-26-14.

Employee was operating a roof bolter, as ee drilled the hole into the mine roof, ee attempted to install the roof bolt using an extension 12" wrench, the wrench broke, striking him in the nose.

Employee was assisting in advancing the high voltage cable for a unit's belt extension. The cable was being transported on a high voltage trailer and was being set into position by a battery bucket jeep. The trailer shifted, causing his foot to be pinched between the trailer and the jeep bucket. He began missing time on 11-24-14.

Employee was pushing a roof bolt into the mine roof, as he was performing this action, he felt pain in his chest area.

Action to terminate citation 9049086: Employee was operating a continuous miner, a piece of foreign material struck EE in the neck.

Employee was installing a roof bolt, as he was pushing the bolt into the mine roof, he felt pain/pop in his wrist.

Employee was installing bolting supplies onto the walk-through bolter, as he was handling the supplies, he felt pain in his back as the area was narrow in which he was traveling.

Employee was drilling a hole into the mine roof to install a roof bolt, as he rotated the drill steels a piece of foreign material fell striking him on the lip. His injury resulted in sutures.

Employee was installing a line curtain to the mine roof and as the coal hauler moved, its cable struck his leg. He began missing time on 9-4-14.

Employee was installing a concrete brattice wall, as ee was lifting blocks loose piece of rock fell from the wire mesh on the mine roof, striking ee on the shoulder.

Action to terminate citation 9049085: Employee was attempting to dislodge a roof bolt from the head of a continuous miner, as ee was pulling on the bolt, a piece of the bolt struck him in the side of the head.

Employee was installing an eight foot roof bolt into the mine roof and as he applied boom and rotation pressure, the bolt stuck his hand. The x-ray results revealed nothing conclusive, he started missing time on 8-29-14.

College temp. employee was taking water line out of a doorway in an arch. As ee was handling materials, ee pinched hand between the water line and coal rib.

Employee was exiting a diesel getman to tighten a ratchet strap, while he was pushing on the ratchet he felt pain in his groin area.

Employee was lifting a belt top frame off of his golf cart, he felt pain in his back as he was lifting. At the request of MSHA, modified to 7 days before returned to work.

Action to terminate citation 9049084: Employee was cutting welding slag, a piece of loose material struck hand.

College temp. employee was walking on an unfinished over-cast to get a sign that was hanging in the way of travel. As ee arrived at the end of over-cast ee lost footing, and fell causing a fracture to ankle.

Employee was building a brattice with another worker. He was putting block up to cap off. The employee handed him a block that was too big and when he went to hand the block back down to be cut he lost his footing.

Employee was walking to a concrete pump, while walking he stepped on a rake causing it to hit him in the mouth, busting his upper lip. He received 3 stitches.

Employee was injured installing a roof bolt into the mine roof, by pushing the roof bolt into the drill hole.

Employee was climbing down off of a pallet of concrete blocks, he had stepped down onto the ground with the left leg, however when he stepped down with his right leg he twisted his knee and felt a pop. When he put weight on his right leg he felt his knee pop twice.

Employee was removing a belt roller and he pinched his finger between the roller and the mounting frame. He received sutures for the laceration.

Employee was plugging up a battery scoop at a charging station, as the plug connected it flashed and caused a burn to his hand.

Employee was operating a walk-through bolter and installing a roof bolt into the mine roof. As he was handling the bolting supplies he pinched the tip of his right index finger between the bolting supplies. He started missing time on 4-10-14.

Employee was handling bolting supplies at the back of the roof bolter the bolter operator let the back end of the bolter down to manage supplies easier. Employee's foot was caught underneath the frame of the bolter pinching it between the frame and the mine floor.

Employee was operating a roof bolting machine and installing a roof bolt in the slider of an unbolted cut. A piece of foreign material fell from the roof striking him on his foot.

Action to terminate citation 9049083: Employee was checking to see if the suction was working on the drill pot of the walk-through bolter, employee dropped the boom and pinched the tip of finger.

Employee was operating a walk-through bolter to install roof bolts into the mine rib outby the last open. A piece of loose rock fell striking him on the finger, causing a laceration which required 6 sutures. At the request of MSHA, modified to 1 days before returned to work.

Employee was moving the continuous miner's power cable from the coal hauler's travelway. As he was handling the cable, he tripped on uneven bottom and fell to the mine floor. He began missing time on 1/20/2014.

Action to terminate citation 9049082: Employee was working on a planetary on a continuous miner, as EE was striking with hand tools, EE was struck with a foreign object.

Employee was assisting the continuous miner operator in moving from place to place. As he was walking back toward the coal transporter, he tripped on uneven mine floor and fell into the coal transporter. During the fall, he injured his left shoulder causing a fracture.

Employee was grabbing a bundle of plates for the pinner, while grasping them he felt his finger pop.

Worker stepped back onto some dried tekseal causing him to fall and twist his ankle

Worker was helping to build a crib, he was capping off the crib and a rock fell hitting him in the back of the head and knocking him down twisting his ankle.

Employee was carrying supplies underground, he slipped and fell striking his left arm. He was taken to the local hospital and x-rays revealed a fracture to his forearm. He began missing time on 8/12/2013.

Employee was operating the continuous miner (CM) and he noticed a large piece of draw rock resting on the top of the CM. He was attempting to remove the draw rock and the rock broke loose, sliding toward the employee and striking and fracturing his left foot. He started missing time on 7/31/2013.

Employee was operating a walk through bolter and as he was installing a roof bolt he felt pain in his shoulder, while pushing the roof bolt into the drilled hole. Starting missing time on 1-2-14.

Worker was walking to retrieve a sledge hammer and there was some uneven ground. Worker twisted his ankle, but there was no fall.

Employee was walking toward the last open crosscut to assist the continuous miner operator in moving from place to place. As he was crossing a coal hauler's cable, he tripped on the cable and fell onto the mine floor causing a fracture to his kneecap. He began missing time on 6-10-13.

Instructed by MSHA to submit the following 7000-1 under duress. A fall of the roof occurred for what we view as a non-reportable roof fall in the 1st N.W. Mains crosscut 33 #7 entry, measuring 15' width by 10' length by 10'-12' height.

Injured was operating chain hoists being used to lift a conveyor transfer chute into place. The upper hook of the hoist came free and released chute to swing in a downward arc contacting the injured shoulder. Employee has all proper training, was wearing safety gear, and was working within his job duty and skill level. No regulations were fouled. No mining equipment involved

Worker was carrying crib blocks (he was cribbing) and tripped over a steel band which caused him to fall and sprain/strain his lower back.

Employee was adjusting and repositioning the forks on a diesel low trac. Unaware that his finger was placed in a pinch point, the employee let go of the fork to allow it to set into position. As a result, he pinched the tip of his left ring finger between the fork and the frame of the low trac.

Employee was tramming a walk-through bolter by remote control, he stepped between the bolter and the coal rib, striking himself with the bolter. He began missing work on 4/15/2013.

Employee was riding as a passenger in an open top mantrip. The front of the mantrip bumper struck some loose belt that was stacked to the side of the entry. The belt caught his left foot/ankle and twisted his foot/ankle as well as his left knee. He began missing work on 4/15/2013.

Worker was tying ribbon to the roof when a piece of foreign material fell and hit him in the head and some of the material fell under his safety glasses into his right eye. Worker was seen by an optometrist who removed two small pieces of metal from eye and he was placed on antibiotic eye drops.

A fall of the roof occurred on the 2nd N.E. Sub Mains #1 unit crosscut 23 #4 entry 3 crosscuts inby the tailpiece, measuring 15' width by 10' length, 8'-9' height.

Employee was attempting to disconnect the water hose from the section feeder, he was unaware that the water hose was currently under pressure from the main water line. While disconnecting the hose, he was struck with the hose causing pain to his left shoulder. He started missing time on Wednesday 3/27/2013.

Employee was drilling into the mine roof with drill steels. While he was applying boom and rotation pressure, the steels became stopped up with drilling dust and the operator did not release the pressure before the steels broke, striking him in the left arm causing a fracture. He started missing time on 3-22-13.

Employee was hanging danger flagging at the 2nd to last row of bolts from the face. While he was standing on the bolting platform (walk-through bolter), the operator activated the T-bar which caused it to collapse at its pivot point, pinching his right foot between the T-bar and the frame of the bolter causing a fracture. He began missing time on 3-18-13.

Employee was assisting the walk-through bolter in moving from place to place. As he was handling the bolter cable, he stepped between the bolter and the cable, which caused the cable to strike him, knocking him to the mine floor. During the fall, he fractured his right ankle. He began missing time on 3-15-2013.

Employee was entering the mantrip and preparing to go underground, he slipped and fell causing a fracture to his right ankle. He started missing time on 2-12-13.

A fall of the roof occurred on the 1st N.W. Panel #4 unit cross cut 38 #7 entry, measuring 15' length by 19' width by 8'-9' height.

A fall of the roof occurred on the 1st N.E. Mains #3 unit crosscut 67 in the #00 entry, measuring 15' width by 15' length by 7'-9' height.

Employee was installing cribs, he lost his footing on uneven mine floor twisting his left knee. He began missing work on 1/17/2013.

Employee was installing timbers into an arch. As he lifted over his head, he felt pain in his shoulder. He started missing time on 2-1-13.

Walking to reclaim belt line and was caught in a rock fall, causing a fracture is right distal tibia and a fracture in the right fibula.

A fall of the roof occurred on the 1st. S.E. Mains #2 unit crosscut 44 #5 entry. Measuring 15' length by 16' width by 7'-8' height.

Removing timbers from pallets and then setting timbers, when he removed one several fell and one hit his left index finger severing the fingertip.

Instructed by MSHA to submit the following 7000-1 under duress. The employee reported an alleged accident that he was on top of a diesel scoop and slipped and fell from the scoop, striking his right hip on the mine floor. On 12-13-12, he began missing work for what we view as a non-work related non-reportable lower back issue, which is not a work-place injury or accident.

Employee was walking down the slope, slipped and fell causing a fracture to his ankle and striking his back on the concrete. Surgery was performed on 12-5-12 to repair his ankle. He began missing time on 12-5-12.

A fall of the roof occurred on the 1st S.E. Panel x-cut 25 #4 belt entry inby tailpiece. Measuring 15' length by 16' width by 8'-9' height.

Employee was walking underground, he stepped on uneven mine floor twisting his left knee. He began missing time on 1/23/2013.

Employee was operating a roof bolter installing a channel for a belt header, as he rotated the bolt, the channel caught on the bolt, striking him on his right wrist, causing a fracture. He began missing time on 10/09/2012.

Employee was operating a roof bolter, his bolting steel hung in the drilled hole, he pushed the wrong lever to remove the steel, causing his steel to rotate striking him on his left elbow causing a laceration.

A fall of the roof occurred on the 11th 54 S.E. LOM Mains C c-cut 511 (supply road). Measuring 20' length by 20' width by 5'-7' height.

A fall of the roof occurred on the 1st 54 S.E. Mains x-cut 44 (belt). Measuring 20' length by 20' width by 5' height.

A fall of the roof occurred on the 6th S.E. LOM Mains B x-cut 140-141 (belt). Measuring 25' length by 20' width by 5'-7' height.

Employee was attempting to install a tram motor for a battery scoop. While lifting the motor with a come-along, the motor slid and struck him in the leg/ankle causing a fracture. He began missing time on 8-17-2012.

Employee was installing cribs in the 9 seam. He was in a high place and a piece of draw rock fell from between pins striking him on the head/neck. He began missing time on 9-14-12.

Employee was walking underground, he lost his footing, causing him to fall. He caught himself from striking the mine floor with his left hand, causing a laceration to his left hand.

Employee was operating a continuous miner. He was cutting in the #5 entry face. He backed the continuous miner up to set over for his second lift cut. He reached down to pick up some supplies that had fallen off the continuous miner and a rock slid off the continuous miner striking him on the right ankle causing a fracture. He began missing time on 7/25/2012.

Employee was walking underground, he stepped on uneven mine floor twisting his knee. He began missing time on 8-24-12.

A fall of the roof occurred on the 8th N.E. Mains at cross cut 106 on 10B, #4 entry (belt). Measuring 30' length by 20' width by 5'-8' height.

Employee was positioned between two (2) parked golf carts, a third golf cart was attempting to move and stuck the side of one of the parked golf carts. His right knee was pinched between the two (2) parked golf carts. No surgery was required. He began missing time 5-24-2012.

Employee was installing crib block for square set roof support. When he attempted to place the crib block into position his left finger was pinched between the crib block and the frame (metal) of the square set, causing a laceration to his finger.
