THERE WAS A MALFUNCTION IN THE STARTUP OF OUR PRIMARY JIG. THE INJURED EMPLOYEE OPENED THE MAIN ELECTRICAL PANEL TO RESET THE BREAKER WITHOUT FIRST SHUTTING DOWN THE POWER SOURCE AND LOCKING O UT. UPON OPENING THE PANEL AN ARC WAS CAUSED BY UNKNOWN REASON & A FLASH FIRE TOOK PLACE. THE EMPLOYEE HAD JUST RECEIVED LOCK-OUT TRAINING PROCEDURE & THEIR OWN LOCKS, 1 WEEK PRIOR. 2ND DEGRE
