While ripping in bulldozer, operator came off of bump/rock resulting in a jarred neck.
Employee slipped on muddy bulldozer tracks getting off machine, twisting and hitting knee. Employee did not miss any work. 10/11/12 received note stating operator should wear elastic brace while working. 10/26/12 received note with a diagnosis of a meniscus tear and a planned surgery. We determined we should report after this diagnosis. Employee has not missed work.
EMPLOYEE WAS PUTTING A ROCK IN A CRUSHER AND THE ROCK CAME DOWN AND PINNED HIS RIGHT HAND BETWEEN WALL AND CRUSHER CAUSING INJURIES TO HIS RIGHT PINKY FINGER.
EMPLOYEE WAS SHOVELING SNOW OFF BULL DOZER EMPLOYEE PULLED MUSCLE IN LEFT ELBOW.
