Emp had a cutting edge of a Cat 980 bucket fall on his foot. Report was not submitted until 10/31/07. Emp missed 1 day of work and was then on restrictions until 11/12/07 when he was released. Info was attempted to be entered via the Internet, but MSHA's online filing was not operational.
Employee was changing tires and swinging a sledgehammer and has a bulging disc. Employee did not report this injury until 8/17/2007.
