Emp had a cutting edge of a Cat 980 bucket fall on his foot. Report was not submitted until 10/31/07. Emp missed 1 day of work and was then on restrictions until 11/12/07 when he was released. Info was attempted to be entered via the Internet, but MSHA's online filing was not operational.

THE SU PERINTENDENT THOUGHT THE INJURY OCCURRED AT HOME, SINCE NOTHING WAS REPORTED ON FRIDAY, THEREFORE NO REPORT WAS EVER FILED.
