Ankle sprain. Info inconsistent, EE settles on: when jumping from 12k telehandler. Why did not use steps? EE states they never do, too much of a hurry. Walks fine at ER waiting. Diagnosis sprain. Refuses x-ray. Refuses to inform ER work related, accepts billing themselves. Given 1 day release. walks fine out of ER. Return to work next day.

Employee states was "not watching where going". Tripped stepping into upstairs control room at dry plant. Caught the fall with left hand. Bent left thumb backwards. Dr diagnosed sprain of thumb.
Dry Plant. Getting down from screener unit after raking off sand buildup. Placed foot on waste conveyor rail. Lost footing, caught fall with left hand. Bent hand back. Dr. diagnosed as hairline fracture in small bone in wrist.

An EE saw IE fall on the steps to the control room. Two EE's checked on IE. IE was shacking and slurring . IE was able to tell them IE's left side was numb. An EE called emergency and Safety supervisor. Another EE took IE to hospital. Doctor told EE that IE was having a mild stroke.
