Employee was walking across a parking lot towards his haul truck when he slipped on a patch of ice and fell, resulting in sprains to his left knee and shoulder and abrasion to his left knee. Employee was placed on three days restricted duty, operator accommodated restrictions - no lost workdays were incurred as a result of this injury.

Injured employee was kneeling in area between the cab and engine compartment on a Cat 988B Loader while performing maintenance to the loader. When employee stood up he heard a snapping noise in his left knee and felt that his left knee had popped out of socket. Initial medical review of this injury resulted in restricted duty with no lost time.

Employee was on scales. He got out of his truck and stepped on the guardrail they have for them to step on to get his load ticket. He turned to get back in his truck when his foot slipped on guardrail and he fell hitting his left side on guardrail and fracturing a rib.
Employee was using 12 volt tester. The tester slipped off temp switch and came in contact with fan blade. The fan blade struck tester and drove blunt end into palm of right hand requiring 16 stitches.

EMPLOYEE WAS HELPING TO INSTALL A SET OF ROLLERS ON THE IMPACTOR CONVEYER. HE TURNED TO GET A PRY BAR, AFTER DOING SO HE BEGAN TO COMPLAIN OF PAIN IN HIS BACK. HE CONTINUED TO WORK HIS SHIFT T HAT DAY. WENT TO THE DOCTOR 1-7-2000, AND WAS DIAGNOSED WITH A STRAINED BACK MUSCLE. TOLD TO STAY OFF WORK A FEW DAYS AND GIVEN PAIN MEDICATION.

EE WAS OPERATING CAT 988B LOADER. HE WENT TO REMOVE A STRAP FROM THE CROSS BAR ON THE LOADER WHEN THE STRAP FLEW LOOSE STRIKING HIS LOWER LIP. THE METAL END OF THE STRAP PUNCTURED HIS LOWER LI P AND LOOSENED HIS 2 FRONT LOWER TEETH.
EE WAS OPERATING A AIR CHUCK AIR BLOWER BLOWING OUT THE AIR FILTERS ON THE IMPACT CRUSHER WHILE DOING SO A SMALL PIECE OF METAL BLEW INTO HIS RIGHT EYE. THIS WAS ON FRIDAY WHEN EE WOKE UP ON M ON AM HIS EYE WAS SWOLLEN. HE WENT TO THE LOCAL CLINIC IN GRAYSON & WAS SENT TO AN EYE SPECIALIST IN ASHLAND WHERE THE METAL PARTICLE WAS REMOVED
EE WAS TIGHTENING IMPACT CRUSHER LOCK DOWN USINGA 2" WRENCH. THE WRENCH SLIPPED MASHING FINGER (RIGHT INDEX) BETWEEN IT & THE BELT COVER. EE WAS TAKEN TO ER WHERE HE RECD 2 STITCHES & THE FIN GER WAS CLEANED & BANDAGED. IT WAS ALSO X-RAYED & FOUND TO A SMALL BONE CHI[
EMPLOYEE WAS PERFORMING HIS PRE-SHIFT EQUIPMENT CHECK ON THE HITACHI 550 EXCAVATOR. WHILE WALKING ON THE CATWALK, HE SLIPPED AND HIS FOOT LODGEDIN THE HAND HOLDER. HE LOST HIS BALANCE AND FELL BACKWARDS ONTO THE TRACK, BREAKING HIS RIGHT LEGIN TWO (2) PLACES.
EMPLOYEE WAS USING A HAMMER AND DRIFT PIN TO REMOVE OLD BUSHINGS OUT OF THE LIFT CYLINDER. THE HAMMER GLANCED OFF THE DRIFT PIN STRIKING THE SMALL FINGER ON HIS LEFT HAND.
EMPLOYEE WAS WALKING DOWN A RAMP AT SCALEHOUSE OFFICE WHEN HE STEPPED ON A SMALL ROCK. THIS CAUSED HIS FOOT TO GO OUT FROM UNDER HIM. WHEN THISHAPPENED IT CAUSED HIM TO SPRAIN HIS LEFT KNEE.
