Arcosa Specialty Materials Seattle #1890 Metal/Non-Metal

Pinch point, right hand laceration (stitches) and chipped bone in right pinky finger.

The accident occurred at the rotary feeder that had stopped rotating. When attempting to get the feeder rotating again, the employee pulled on the drive chain and subsequently got hand caught between the drive sprocket and the chain.

Employee was carbon arcing inside the materials dryer. A piece of rolled off the welding jacket to EE's groin area causing burns to EE's genitals.

EE was doing an elevator chain and bucket inspection after the elevator had been repaired. EE was sitting on a bucket outside of the elevator, controlling the chain with an inverter and looking up when a 1in -rock became dislodged from the chain/bucket above and struck in the eye. EE was not wearing any eye protection.

EE was moving piece of metal when it slipped and made contact with EE's mouth, requiring a couple of stitches.

EE was stepping off ladder onto uneven surface, and felt a pain in foot (i.e. Pulled muscle or sprain).

EE was cleaning mud off of belt when tool caught on belt and injured finger, requiring stitches.

Was grinding on metal when a small piece went in his eye. Had to have eye flushed to remove it.

Warehouse worker was filling a bag with product at a sacking station. He bent down to adjust the bottom of the bag when it made contact with his eye, causing a superficial abrasion to the cornea.

Pulling on a chain come-along and something got in his eye (piece of metal).

Climbed down ladder, stepped wrong onto pavement and rolled ankle over.

A coworker had moved a chain that had become wrapped around a ladder that fell and hit EE on the hardhat and right shoulder

A cylinder was lodged in the welding cart. In trying to move/dislodge cylinder against advice, miner injured back muscle.

Victim was securing equipment from loading dock (warehouse) to truck, when he lost his balance and fell approx 4' to the pavement. Left ankle sprain.

Victim was hammering on a bearing role to loosen it wehn a metal bit flew into his left eye. victim was wear glasses but not safety glasses.

Warehouseman placed tool on floor and then stepped on it, twising and spraining his right ankle.

EMPLOYEE WAS KNOCKED DOWN BY A FORKLIFT BACKING OUT OF THE BAY. EQUIPMENT RAN OVER HIS FOOT. BACK-UP ALARM WAS FUNCTIONING PROPERLY. CONTUSION TO FOOT AND THIGH. SPRAINED ANKLE.

WHILE REARRANGING SOME STEEL PLATES BEING TRANSPORTED BY A FORKLIFT, ONE PLATE FELL, STRIKING EEAND SLAMMING HIM INTO A CEMENT WALL RESULTING INCONTUSIONS ON BACK & ARMS & FRACTURED RIB.

CHANGING HAMMERS IN HAMMER MILL AND FINGER GOT CAUGHT BETWEEN A METAL BAR AND A BEARING CAP. RESULTED IN CUT ON LITTLE FINGER REQUIRING STITCHES, AND LOSS OF FINGERNAIL.

EE STOOD ON A ROUND PIPE IN ORDER TO REACH SOMETHING. HIS FOOT SLIPPED OFF THE PIPE & HE FELL TO HIS RIGHT, BRUSING HIS RIGHT HIP.

EE WAS WORKING REPAIRING ON ,MILL ACCESS DOOR WHEN THE DOOR UNEXPECTEDLY MOVED EE LURCHED OUT OF THE WAY AND LOST HIS BALANCE FALLING 5 FT TO THE FLOOR,LANDING ON HIS RIGHT SIDE.

INJURED WAS STANDING NEST TO PALLET & TALKINT TO OTHER EMPLOYEES FORKLIFT DRIVER ENTERED PALLET WITH FORKS NOT SEEING IN THE EMPLOYEES POSITION & RAN INTO FOOT WITRJH FORKLIFT FORKS

INJ WORKMAN WAS STANDING TOO CLOSE TO A BACKING FORKLIFT AND THE TIRE WENT OVER THE BACK OF HIS FOOT.CONTUSION ON RIGHT FOOT.

WHEN THE WAREHOUSEMAN DISMOUNTED FROM A FORKLIFT, HE FAILED TO SET THE EMERGENCY BRAKE. UNNOTICED, THE FORKLIFT SLOWLY PINNED THE MAN AT HIS KNEE.
