A/C vent cover fell off of the 988F wheel loader. Employee was attempting to put the A/C vent cover on. They stuck their fingers into the vent hole, the fan was still running and a finger came into contact with the steel fan blades which cut the fingers.

EE was working in secondary plant inspecting & greasing when he began to feel ill. He went to employee break room to sit for a few minutes but felt no better so he went to office. Plant manager took him to nearby med. clinic. The doctor suspected a heart problem & had him transported to Wake Med. hospital. He was kept, tested & released on 06/09 with no diagnoses & no restrictions.

AN OUTSIDE CARRIER BROUGHT IN A STEEL STRUCTURE FOR THE NEW PLANT EXPANSION. IN AN ATTEMPT TO HELP THE DRIVER, EMPLOYEE RELEASED THE CHAIN BINDERS OFF OF THE STRUCTURE. WHILE RELEASING ONE OF THE CHAIN BINDERS WITH A 2-3' PIPE, THE CHAIN BINDER BROKE AND STRUCK HIM BETWEEN THE EYES.
A MECHANIC WAS WORKING ON A HAUL TRUCK WHEN A PRESSURIZED HYDRAULIC LINE CAME LOOSE,HITTING THE MECHANIC IN THE HAND, INJECTING HYDRAULIC FLUID INTO THE LEFT INDEX FINGER.

EE WAS TUBING HOLES THAT HE HAD DRILLED. HE HAD PUT TUBE IN HOLES AND WAS CUTTING THE TOP OF THE TUBE LEFT OUT OF HOLE WITH A RAZOR KNIFE. HE WAS PUSHING THE KNIFE AWAY FROM HIM AROUND THE TUB E. HE TURNED THE KNIFE AROUND WAS PULLING BACK AT HIM AND WHEN HE GOT TO WERE HE HAD CUT, THE KNIFE SLIPPED AND WENT INTO HIS RIGHT LEG.

EE WAS OBSERVING OPERATION OF DRILL, WHEN THE OPERATOR LET JACKS DOWN OF UNLEVEL GRADE, CAUSING DOOR TO SHUT ON EE'S RING FINGER ON LEFT HAND, CUTTING THE TIP OF HIS FINGER.

TWO EE'S WERE LOADING SECTIONS OF PIPE (10'SECTIONS - 120#), INTO LOADER BUCKET. AS HE LET GO OF PIPE, HIS LITTLE FINGER BECAME ENTANGLED BETWEEN LOCKING RING AND PIPE. THE PIPE ROLLED OVER CA USING SEVERE DAMAGE TO LITTLE FINGER AND RING FINGER ON RT HAND.
EMPLOYEE WAS IN CAB OF DOZER WORKING BLADE TO HELP REMOVE A RIPPER. AFTER THEY HAD FINISHED, HE WAS CLIMBING DOWN OUT OF CAB, WHEN HIS FOOT SLIPPED. HIS LEFT LEG FELL BETWEEN THE TRUNION ARM AND TRACK FRAME TRAPPING IT ABOUT KNEE LEVEL. EMPLOYEE WAS HAVING SEVERE PAIN IN LEFT KNEE AREA AND SOME PAIN WITH TOES.

EE WAS WORKING ON A JAW JAM TRYING TO REMOVE A BIG ROCK. HE WAS IN THE HAW PUTTING A CABLE AROUND IT WHEN A ROCK FROM OUT OF THE FEEDER ROLLED OUT OF THE FEEDER INTO THE JAW AND LANDED ON EE'S FOOT. BROKE LITTLE TOE ON LEFT FOOT.
EE INJURED HIS LEFT THUMB WHEN TAKING IDLER OFF OF 30P CARNE. HE WAS USING A CROW TO PUSH THE IDLER OUT AND CLEAR OF THE TRACK FRAME. WHEN THE IDLER FELL, IT CAUGHT HIS THUMBE BETWEEN THE FRAM E & IDLER. HIS THUMB HAS AN ABRASION-SWELLING. WE SENT HIM TO THE DR. FOR X-RAYS. ON FIRST X-RAYS THERE WERE NO FRACTURES. HE WENT BACK ON THURSDAY AND THEY FOUND HE HAD BROKE HIS LEFT THUMB.
