A maintenance worker was completing prep work for the installation of a belt scale when his tool kicked back and made contact to his left forearm.
Utility Operator while shoveling off a truck on the overload platform felt a pop in tendon of his hand.
Miner felt a sting in his left shoulder. Administered first aid: ice packs and heat treatments. On 3/20/14 employee mentioned the first aid was not working enough. He was sent to occupational health clinic for evaluation. He was released with some restrictions and requires physical therapy. This makes it a medical treatment injury with no lost time days.

Prod Supt received voice mail message from employee on Sunday 10/20/13 that employee was going to the doctor for eye irritation. Metal had to be removed from eye at a local medical facility. No incident or specific event was reported on employee's last shift 10/18/13. The incident is still being investigated.
Employee pinched left little finger while setting steel
IE was installing conduit in a 18" trench for temporary power to a contractor's job trailer when he went to step out of trench he felt pain in his right lower calf. IE was transported to medical facility where he was diagnosed with a muscle strain and partial tendon rupture. IE was prescribed a pain reliever and given restrictions to not climb ladders.
While working on an auger employee twisted body and felt his back shift.

While inspecting overhead water lines employee fell causing right leg to get scraped and a small puncture wound just above knee.
Employee was struck on the head by a cluster of sand that fell from an overhead conveyor.
Employee slipped on some sand and fell backwards causing a sternal fracture.

EE had seizure causing same level fall, causing him to sustain laceration requiring stitches. MD report states seizures are medical condition, not occupational injury/illness. This will be treated as non-recordable injury.
Employee mis-stepped, tripped and fell while climbing stairs.
