I HAD LOADED MY LAST BUCKET OF ROCK IN THE LOADER & BACKED UP IN FIRST GEAR TO LINE UP WITH MY TRAILER. WITH THE BUCKET HALF RAISED I SHIFTED THE LOADER FORWARD WHILE STOPPED. LOADING WAS TAKI NG PLACE ON A SLIGHT HILL. AS I BEGAN TO PROCEED FORWARD I BEGAN TO RAISE THE BUCKET TO EXTEND IT OVER THE SIDE OF THE TRAILER. THE LOADER TIPPED ON THE PASSENGER SIDE WHERE IT CAME TO REST.

EXITING HAUL TRUCK. LEFT HAND SLID, ALL WEIGHT SUPPORTED BY RIGHT HAND, EMPLOYEE FELT SOMETHING SNAP, WRIST STARTED TO SWELL.
THE WORKER WAS HIKING UP A HILL IN THE SOUTH EAST CORNER OF A QUARRY TO DISCUSS AND START TO LAYOUT STRIPPING OPERATIONS. THE HILL WAS VERY STEEP AND IN THE PROCESS OF HIKING UP, THE MUSCLE IN THE LOWER LEG KNOTTED UP AND THEN TORE.
WHILE CLIMBING IN LOADER, EE TURNED TO SIT DOWN, FOOT HUNG UP ON FLOOR MAT, TWISTING KNEE.

WHILE REMOVING THE OUTSIDE REAR DUAL TIRE OFF OF TRAILER 101, EE REACHED UNDERNEATH THE RIM TO PULL OFF. DURING THIS, BACK WAS STRAINED AND THE PAIN INCREASED THROUGHOUT THE DAY. BY 0130 THE N EXT DAY, THE PAIN HAD INCREASED TO THE POINT MEDICAL ASISTANCE WAS NECESSARY.
THROUGHT OUT THE DAY WHILE WORKING ON A 1984 KENWORTH UTILIZING A PLASMA CUTTHER. APPARENTLY THERE WAS ENOUGH OF A FLASH TO CAUSE BURNING OF EMPLOYEE'S RETINAS THROUGH THE OUTSIDE CORNERS OF S AFETY GLASSES.

WHILE MOUNTING TIRES, EE REACHED UP TO GRAB THE VALVE CORE AND THE JET RING SOMEHOW GOT TURNED ON AND FLEW UP AND HIT HIM IN THE UPPER RIGHT CHEEK BONE AREA (BELOW THE EYE). EE WAS NOT BLEEDIN GSO HE SAT DOWN FOR A FEW MINS THEN WENT BACK TOWORK. AT ABOUT 11:45AM EE BEGAN FEELING DIZZY AND GOT A HEADACHE. WAS TRANSPORTED TO THE EMERGENCY ROOM BY SUPERVISOR.

WHILE TIGHTENING A U-BOLT ON A BELLY DUMP TRIALER, THE ACK HOLDING THE AXLE UP FELL DOWNWARD (RIGHT UPPER PORTION) HITTING EMPLOYEE'S LIP.
WHILE EE WAS WALKING BEHIND A CRANE TRUCK, HE SWUNG HIS LEFT ARM, HITTING HIS LEFT HAND ON THE TOMMY LIFT PROTRUDING FROM THE BACK.
WHILE ROLLING RAIL CARS DOWN DURING THE LOADING PROCESS, EE WRENCHED DOWN ON THE RAIL CAR BRAKESTO STOP THE CARS. THIS CAUSED A SHARP PAIN IN HIS LEFT CHEST AREA. THE PAIN BECAME PROGRESSIVELY WORSE EACH DAY UNTIL MEDICAL SERVICES WERE SOUGHT ON 1/14/97.
WHILE OPERATING THE BOBCAT LOADER, MY HEAD/HARD HAT WOULD HIT THE HEADACHE RACK. AFTER ONE SHIFT, MY NECK STARTED HURTING UPON ARISING IN THE MORNING. (SEE ATTACHED LETTER OF EXPLANATION)

WHILE OPERATING A CAT 980 LOADER, DIGGING DIRT OUT OF THE BANK TO FILL THE KOLBERG SYSTEM, A LARGE FROZEN WAD OF LIMESTONE FELL FROM THE BANK. THE WAD OF LIMESTONE CAME THROUGH THE WINDSHIELD, HITTING EMP IN THE CHEST.
EE WAS WALKING THE LENGTH OF A TRAILER, WEARING HIS SAFETY BELT, IN ORDER TO LINE UP TRUCK SPOUTS WITH THE DOMES ON THE TRAILER. WHILE LOOKING AT THE FRONT DOME, HE STEPPED INTO THE REAR DOME WITH HIS LEFT LEG CAUSING A FALL INTO THE DOME.
EE WAS BREAKING UP A CLOD OF DIRT AT THE BOTTOM OF THE BGI LOADOUT SYSTEM. THE SHOVEL SPADE WAS POSITIONED BETWEEN THE CLOD AND THE LOADOUT LIP. WHEN THE CLOD LOOSENED, IT FORCED THE SHOVEL BA CK, HITTING EE ABOVE THE RIGHT EYE.

I WAS WORKING ON THE MAIN ELEVATOR AT MONTANA LIMESTONE, CO. WHEN WE DECIDED TO BUILD A CAGE AROUND THE TOP TO PROTECT US FROM THE WIND AND SNOW. THE CAGE CONSISTED OF SQUARE STOCK TUBING THA T HAD TO BE WELDED TOGETHER. AFTER WELDING TO CAGE TOGETHER WE CONTINUED TO REPAIR THE ELEVATOR, WHEN I WENT HOME THAT NITE THERE WAS PAIN, ITCHING, SWELLING OF BOTH MY EYES. LATER DIAGNOSED

HE WAS WALKING DOWN THE WALKWAY IN A TUNNEL (THAT WAS BEING REMOVED) TO CHECK ON A PUMP MOTOR WHEN THE WALKWAY FELL OFF THE SUPPORT. THE WALKWAY WAS ABOUT 4 FEET ABOVE GROUND WHEN WALKWAY FELL . HIS KNEE GAVE OUT.
