D11R operator contacted me by CB approx 11:20 PM stating that he had injured his back climbing on his dozer at worktime (5:30PM). He said the pain had worsened as the shift progressed. I took him to Whitesburg ARH. The x-ray was negative. They said he may have pulled muscle or strain. He was advised to see family doctor on April 7, 2009

EE stepped down from grease truck, in doing so he turned his ankle on a piece of frozen dirt.
Operator was climbing down from 988 loader when he missed the bottom ladder rung or step causing him to turn his ankle when his foot made contact with the ground. He was taken to the A.R.H. ER where it was determinged he had sprained his ankle. He returned to work on 10 Nov 08 without missing a shift.
EE informed me at 2100 that he had injured his back getting on his equipment (992G) at work time. I then took EE to ARH (ER) for drug test and whatever medical treatment was needed.
Employee stated he dismounted D10 to use bathroom and was climbing back on the tractor when he slipped injuring his knee. He had knee problems prior to accident.
Employee was checking oil on D10N when he lost his footing during heavy rain. The grab rail caused him to twist possibly causing strain or back injury.

VICTIM STEPPED IN A DEPRESSION WHILE WALKING ACROSS EQUIPMENT PARKING LOT. PULLED TENDONS IN THE BOTTOM OF FOOT.
OPERATOR STATES THAT HE BACKED THE DOZER OVER A SMALL ROCK AND IT JERKED HIM CAUSING PAIN IN LOWER BACK AND LEGS. STRAINED MUSCLE.
OPERATOR RAN OVER A ROCK WITH DOZER AND JERKED HIS BACK STRAINED MUSCLE.
EMPLOYEE WAS USING A DISC GRINDER TO REMOVE WELDING SLAG. THE GRINDING DISC EXPLODED AND STRUCK BOTH LEGS BELOW THE KNEES. LACERATIONS TO LOWER LEGS REQUIRING STITCHES.
WHILE PUSHING A LARGE ROCK, THE BLADE SLIPPED AND THREW HIS RIGHT SHOULDER BACK AGAINST THE SEAT. POSSIBLE PINCHED NERVE OR JAMMED SHOULDER.
VICTIM WAS WELDING WHEN A PIECE OF SLAG GOT BEHIND SAFETY GLASSES IN RIGHT EYE. SCRATCHED EYE, OINTMENT APPLIED.
OPERATOR SAYS USING THE RIPPER CAUSED HIS BACK TO HURT.

HE WAS DESCENDING DOZER TO TALK TO THE FOREMAN & SLIPPED OFF PUSHARM & FELL TO THE GROUND ON HIS LEFT SIDE. THE WEATHER CONDITIONS WERE WET & VERY MUDDY. HE WORKED THE REST OF THE DAY. HE WORK ED THE NEXT DAY UNTIL 1:00 P.M. & CALLED THE OFFICE FOR A DOCTOR'S APPOINTMENT. HE WENT TO THE CLINIC FOR X-RAYS.
HE WAS HITCHING THE STEAM JENNY TRAILER TO THE FOREMAN'S PICKUP TRUCK AND CAUGHT HIS RIGHT MIDDLE FINGER BETWEEN BALL & HITCH. HE WENT TO THE E.R., GOT X-RAYS AND TREATMENT. HIS FINGERNAIL HAD BEEN PARTIALLY TORN OFF. THEY LEFT IT ON THE FINGER AND APPLIED BANDAGES.
WET & MUDDY & RAINING AND HE WAS CLIMBING DOWN THE DOZER TRACKS AFTER SERVICING THE EQUIPMENT. HIS BOOT SLIPPED OFF THE TRACK & HE HAD TO JUMP LANDING ON THE GROUND. HIS RIGHT KNEE FOLDED UNDE R HIM. HIS KNEE STARTED TO SWELL AND HE DROVE HIMSELF TO THE A.R.H. CLINIC IN HAZARD - X-RAYS.
EE SAYS HE SLIPPED ON THE STEP OF THE WATERTRUCK& FELL BACKWARD ON THE GROUND. THERE WERE NO WITNESS. HE CONTINUED TO WORK FULL TIME. CHOVIESCLINIC NOTIFIED US 10-07-02 THAT HE WOULD BE OFF WORK UNTIL FURTHER NOTICE. THE DR. HAS REQUESTEDAND MRI.
WHILE HE WAS DUMPING A LOAD OF ROCK HE WAS DRIVING A ROCK TRUCK A ROCK CAUGHT THE CORNER OF THE TRUCK BED JERKING THE TRUCK AND CAUSING PAIN TO THE RIGHT LOWER BACK.
EMPLOYEE WAS BREAKING A SUB AWAY FROM THE DRILL STEEL ON A DRILL WITH A 12 LB SLEDGE HAMMER AND PULLED HIS LEFT SHOULDER. HE WENT TO THE DR OFFICE AND THEY GAVE HIM MUSCLE RELAXER.
HE WAS STEPPING DOWN FROM THE BULK TRUCK AND CREELED HIS RIGHT ANKLE. IT LEFT HIS ANKLE BRUISED & SWOLLEN. HE WAS TAKEN TO ER FOR XRAYS. HE WAS REFERRED TO PHYSICAL THERAPY FOR 3 VISITS.
HE WAS SERVICING THE EQUIPMENT & HE STARTED DOWNTHE LADDER & HIS FOOT SLIPPED CAUSING HIM TO TWIST HIS LT. ANKLE. HE WAS TAKEN TO THE E.R. FOR X-RAYS & HE WAS GIVEN CRUTCHES. IT WAS TOO SWOLLEN TO DISTINGUISH IF IT WAS BROKEN.
HE WAS FUELING EQUIPMENT A GRADER AND WE WAS TOLD THAT HE BUMPED HIS SHIN BONE. HE SENT THE OFFICE AN EXCUSED FROM DR. TO CORNETTSVILLE DATE 1-4-02. AND DUE DATED 1-10-02. WE DONT HAVE AN OFFI CIAL DATE OR TIME OF THE ACCIDENT. WE GO THE INFORMATION FROM EE.

HE WAS IN A GRADER BACKING UP AND HIT A ROCK AND HE SAID HIS HEAD HAD HIT THE GLASS BUT HE KEPT WORKING. THEN AT 10:30 AM HE WAS GRADING THE ROAD & HIT ANOTHER ROCK JOLTING HIS NECK. HE LEFT T HE JOB & WENT HOME. HE CALLED THE OFFICE ON NOV 20 AND SAID HE WAS OK. THEN ON NOV 21 HE CALLED THE OFFICE & WENT TO FAMILY DOCTOR.
HE WAS ON A 992 USING AN AIR IMPACT WRENCH TO TAKE OFF A SUCTION LINE WHEN THE CLAMP FLEW OFF THE AIR HOSE AND PIERCING THRU HIS LEFT EYE LID INTO HIS EYEBALL. HE WENT TO ER AND WAS REFERRED TO DR.
HE WAS CLOSING THE DOOR ONT HE TOOL BOX THAT WAS ONT HE UPPER LEVEL OF TOOL TRUCK AND IT FELL BACK DOWN. STRIKING HIM ON THE HOSE BRIDGE GASHING IT OPEN. HE WENT TO SHITSBURG E.R. AND RECEIVED TREATMENT. THE DR GLUED THE CUT TOGETHER.
EMPLOYEE WAS CUTTING OFF A ZIP TIE FROM A METAL LINE WITH A POCKET KNIFE. THE KNIFE SLIPPED & HE STABBED HIS LEFT FOREARM JUST ABOVE THE WRIST. WENT TO WHITESBURG HOSPITAL ER & RECEIVED 4 STAP LES.
