Employee was standing on a ladder cleaning a pin to put bushing in tailgate of the Terex TA400. EE was stepping down the ladder, and was further up than they realized. As EE was stepping down to concrete floor, they ended up falling to concrete floor, landing on the left hip - braking a hip bone, with possible injury to shoulder, elbow, head, fingers.

EE was sitting on loader waiting for another truck to be loaded. He broke out in a sweat and had severe chest pain. He had a heart attack.
EMPLOYEE WAS CHANGING OTR TIRE USING BOOM TRUCK, HAMMER, BARS, JACK STANDS. HE WAS PICKING UP A FLANGE FROM THE GROUND, WEIGHING 80-90 POUNDS, AND PLACED IT ON A CAT LOADER

On 6/17/09 EE was bush hogging when he ran into the ditch causing the steering wheel to jerk - jarring his right shoulder. He continued to work until Dr scheduled surgery on 8/27/09.

Employee was loosening a bolt with a wrench to remove a bucket from loader. He used his hand to hold up bolt, spun and cut hand - right palm - had 13 stitches.

Employee stepped up on the D25#7 Water Truck. His foot slipped off the truck and he used his left hand to grab a bar on the water truck to keep from falling, which caused pain in his left shoulder.

EE was swinging a 12 lb sledgehammer to remove a bucket pin from a bucket. He felt a strain or pull in his left elbow.
Employee lifted a screen off the Gradall to put it on the shaker. As he was raising the screen to put it on the shaker, he almost dropped the screen. Bracing the screen to get a better grip, it hurt/twisted his back.

EMPLOYEE WAS REMOVING A GEAR FROM THE PIONEER JAW. THE GEAR SLIPPED FROM HIS HAND AND HIS 5TH FINGER WAS PINCHED BETWEEN THE GEAR AND I-BEAM, BROKE FINGER.

EE WAS WELDING ON THE PIONEER FEEDER. THERE WAS ANOTHER EE BEHIND HIM GRINDING AND SOME OF THE DUST BLEW UP UNDER HIS WELDING HOOD AND GOT IN HIS EYE.

EE WAS TRYIG TO DRIVE A PISTON NTO A DTH HAMMER WHEN A PIECE OF METAL CAME OFF OF THE SLEDGE HAMMER AND LODGED IN HIS UPPER ARM. DIDN'T NOTICE UNTIL 4 HRS LATER WHEN ARM WENT NUMB AND STARTED BLEEDING. WENT TO HOSPITAL WHERE THEY REMOVED A PIECE OF METAL APPROX THE SIZE OF A DIME. REQUIRED 17 STITCHES TO SEW UP.

HAD BEEN WORKING ON THE 1135 KICK OUT BELT. WORK HAD BEEN COMPLETED AND HE WAS COMING DOWN THE LADDER ATTACHED TO THE PLANT. HE EITHER MISSED A STEP OR HIS FOOT SLIPPED OFF THE STEP AND CAUSED HIM TO FALL TO THE GROUND. HE LANDED ON A CONCRETE FLOOR CAUSING HIS RIGHT LEG TO BREAK.

INJURED WAS CHANGING SCREENS ON THE 6 X 20 WHEN HE KNELT DOWN HIS KNEE WAS JABBED BY A BOLT. IT DID NOT BREAK THE SKIN BUT WENT BEHIND THE KNEE CAP,
EE WAS HELPING OTHER CHANGE SCREENS ON THE MESOBI SHAKER. OTHER HAD REMOVED THE DRIBBLE SPOUT AND HANDED IT TO EE TO PUT DOWN. WHEN EE WENT TO LAY IT DOWN HE TWISTED AS HE WAS PUTTING PUTTING DOWN AND CAUSED HIS BACK TO CRAMP UP. HE SADI ITFELT LIKE HE HAD A CHARLIE-HORSE IN HIS BACK BUT IT STOPPED HURTING AND HE CONTINED TO WORK THE REST OF THE DAYS.
HELPING TO BREAK DOWN A HAMME RBIT ON A DRILLING RIG, THE INJURED GOT TOO CLOSE TO THE HAMMER BIT AND PINCHED THE SKIN ON HIS SHOUULDER. THE PINCHIN GACTION CUT THE SKIN REQUIRING STITCHES.
CHECKING TO SEE IF BUMP BLOCK'S WERE IN PLACE. WHEN HE PULLED HIS HAND OUT HE CUT IT ON WATER HOSE BAND CLAMP.
