Employee stated had climbed down onto belt #1 to take pry bar to belt wipe on belt #2 to get unstuck. As EE was climbing out off belt #1, EE reached for the hand rail on the catwalk. EE swung legs toward catwalk and when EE did, felt a pop and pain in low right side back area. Went to get checked out and missed work starting 12/13/18.

Employee stated that he was mounting the loader. His left foot slipped off of the second step. He slid down the steps onto the ground while holding onto the hand rails. Remounted loader and proceeded to work. Felt pain in left leg later. Went to doctor, checked out and returned to work. Became reportable when he missed work on 8-24-09.

Employee stated that he was starting down the ladder on the loader. When he grabbed for the right hand rail, he missed it. His left hand was on the left rail and caught his body as he slid down the ladder onto the ground. He felt a pop in his left shoulder.

Employee stated that he was coming down steps at the #1 vibrator at the stoker plant. He stepped down onto the ground and onto a lump of coal which caused him to roll his left foot over resulting in sprain. Reportable due to restricted work activity.
Employee stated that he was lifting a coal sample off of the tailgate of his pickup truck. He lifted and turned to set the sample beside the screening machine when he felt back pain. Reportable when lost time was incurred on 3-1-07.

Employee stated that he was splicing a conveyor belt. The utility knife which he was using to cut the belt slipped off of the belt and struck his leg causing a laceration to his right leg requiring staples to suture the laceration.
Employee had riffled down and split a coal sample using the coal splitter. He knelt down to pull out the drawer containing the coal sample. When he bent over to pick up the drawer and rose back up, he felt a pop in his back. Persistant pain occurred and he went to the doctor on 3-9-04 which is his first lost workday.

EMPLOYEE STATED THAT HE WAS DROPPING RAILROAD CARS REQUIRING HIM TO TURN THE WHEEL SO AS TO TIGHTEN THE BRAKES TO STOP THE CARS. HE BEGAN FEELING PAIN IN HIS LOWER BACK WHICH CONTINUED THROUGH OUT THE DAY. BACK PAIN PERSISTED DURING THE WEEKAND HE MISSED WORK AND WENT TO THE DOCTOR ON 1-12-01. LOST TIME IS INCURRED.

HE STARTED TO DUCK HIS HEAD UNDER A METAL BAR ON THE RAILROAD CAR TO GO UNDERNEATH IT. HE RAISED UP HIS HEAD TOO QUIK AND HIT HIS HEAD AGAINST THE METAL BAR RESULTING IN A CERVICAL STRAIN.

EE STATED HE WAS CLOSING THE CAR DOORS ON RAILROAD CARS WITH A PRY BAR. AS HE WAS PULLING ON THE PRY BAR TO CLOSE A RAILROAD CAR BOTTOM, THE PRYBAR SLIPPED OFF OF THE DOOR LATCH CAUSING HIM TO LOSE HIS BALANCE. HE JERKED HS BACK WHEN THE BARSUDDENLY CAME LOOSE AND THEN FELL BACKWARDS ONTOTHE GROUND. FIRST LOST WEEK DAY OCCURRED ON 3/24/98
EE STATED THAT HE WAS LIFTING COAL SAMPLES TO PUT INTO THE COAL CRUSHER SPAS TO GRIND THE SAMPLES. NO IMMEDIATE PAIN WAS FELT BUT SHORTLY THERE AFTER THE EE BEGAN EXPERIENCING LOWER BACK PAIN. HE CONTINUED WORK AND DECIDED TO GO TO DR ON 1-11-98. INJURY BECAME REPORTABLE ON 1-12-98 DUE TO LOST WORK DAYS.

THE EE HAD GOTTEN ONTO THE FEED BELT CONVEYOR SO AS TO LOOK DOWN INTO THE CRUSHER, THE EE WAS IN A ST OPPED POSITION LOST HIS BALANCE AND FELL BACKWARDS ONTO THE CONVEYOR BELT CAUSING A CONTUSIONAL STRAIN TO HIS LUMBAR AREA.

EMPLOYEE WAS GETTING ONTO A SMALL CAR 1840 UNI-LOADER TO MOVE. AS HE STEPPED ONTO THE LOADER BUCKET, HIS RIGHT FOOT SLIPPED. HE MOVED HIS LEFT FOOT TO MAINTIAN HIS BALANCE AND AS HE DID, HIS L EFT FOOT STRUCK THE FRONT OF THE LOADER BUCKET CAUSING CONTUSION TO THE TOP OF HIS LEFT FOOT.
INJURED WAS ASSISTING IN CLOSING AND LOCKING THEBOTTOM DOORS ON THE RAILROAD CARS, AS HE WAS PULLING ON THE PRY BAR IN CLOSING THE DOOR, THE BAR SLIPPED OUT STRIKING THE INJURED'S LEFT LEG CAUSING INJURED TO FALL RESULTING IN FRACTURED LEG. INJURED IS UNSURE WHETHER THE PRY BAR OR THE SUDDEN PRESSURE ON LEG WHEN BAR SLIPPED CAUSED THE FRACTURE.
