Employee was in the process of going to shut off impactor at ground level. When they heard a pop in their left leg which caused pain and loss of motion in it. Employee was unable to put any body weight on leg after this. Employee was taken to hospital by management.

While removing a flat rock that was wedged in rock box above jaw belt. Employee was swinging down on rock with a sledge hammer when they heard a pop from lower back. Which in turn caused them extreme pain and discomfort when trying to move. Employee was transferred by EMS to hospital.

While adjusting the setting on jaw crusher a mechanical failure occurred on the adjusting ram hydraulic hose. The failure allowed the jaw adjusting mechanism to close on the employee hands in the adjusting shim area. Employee was transferred by EMS to hospital trauma center with crushing injuries to both hands.
Employee was walking by the rear of welding service truck and contact a piece of sharp metal protruding from the bed. Employee's left upper arm was lacerated requiring sutures.

Employee was replacing bolt-on cutting edge on CAT 980H bucket. The worn plow bolt twisted under the power of removal with air impact. The bolt head caught and cut the employee leather glove lacerating left index finger

EE WAS USING ADJUSTABLE WRENCH TO REMOVE TORCH FROM OXYGEN/ACETYLENE SUPPLY HOSES. THE PULLING MOTION INDUCED TENSION ON LEFT SHOULDER RESULTING IN MUSCLE/TENDON STRAIN. ON 7/28/03, ORTHOPEDIC SPECIALIST PRESCRIBED ANTI-INFLAMMATORY AND POSSIBLE THERAPY.

EE WAS ADJUSTING FORKS ON SKIDSTEER LOADER FORK ATTACHMENT. HE WAS USING RIGHT HAND TO PIVOT ROD WHILE USING LEFT TO GUIDE FORK ALONG PIVOT ROD. FORK SLIPPED OUT OF RIGHT HAND AND PINCHED LEFT DISTAL RING FINGER AT ROTATING PINCH POINT. EMPLOYEES GLOVES WERE OIL SOAKED AT TIME OF ACCIDENT.
EQUIPMENT OPERATOR WAS DISMOUNTING THE CAT 245 HYDRAULIC SHOVEL AT THE END OF SHIFT. WHEN HE CLOSED CAB DOOR, HIS LEFT DISTAL FOURTH FINGER WAS PINCHED IN THE DOOR JAM. FINGER SUSTAINED LACERA TION/CRUSHING INJURY REQUIRING SEVEN SUTURES. NO LOST WORK DAYS.

HE WAS ASSISTING I.NAME IN REMOVING A COMMERCIAL PROPANE TANK BY CHAINING IT TO THE BUCKET OF A HOUGH 80B FRONT END LOADER. THE RING FINGER OF HIS LEFT HAND WAS CAUGHT BETWEEN THE CHAIN AND TH E LIFTING EYE ON THE TANK, CAUSING A LACERATION.HE WAS TRANSPORTED BY COMPANY VEHICLE TO ST ANTHONY'S ER WHERE DR CLOSED THE WOUND WITH SUTURES AND DRESSED IT.
EMPLOYEE WAS WORKING ON A LADDER IN THE SECONDARY PLANT. THE LADDER'S BASE BECAME UNSTABLE, CAUSING THE EMPLOYEE TO FALL APPROX. 10 FEET TO THE NEXT LEVEL. EXAMINATION BY DR. REVEALED A FRACTU RE OF THE LARGE BONE IN THE LEFT WRIST.

THE ROCK WAS APPROX. 6"LX4"W. THE EE FINISHED HIS SHIFT A COMPLETE SHIFT THE DAY AFTER. EE SOUGHT TREATMENT ON 66-22-96. IT WAS DETERMINED HE HAD SUSTAINED A FRACTURE TO THE DISTAL LEFT FIBULA.
