After greasing product elevators, employee lost footing on 6' fixed ladder and fell onto platform below. Employee was taken to hospital and treated for laceration to right middle finger (8 stitches) and hairline fracture to left elbow.
Welding overhead on inlet feed chute for drum. Was wearing safety glasses and welding helmet. Had small amount of slag fall into the helmet. Continued working and finished task. Later, started having irritation and watering of right eye. Sought medical attention and it was discovered that welding slag had became lodged in eye. Surgery required to remove all pieces.

Employee was placing wheel chock on chock hold on 980 loader, when his right hand slipped causing his left thumb to become caught between the chock and the chock holder. Resulted in fracture to employee's left thumb.

Employee was repairing belt skirting along side of an elevated belt. As he stepped down the ladder the ladder fell to one side resulting in the employee falling to the ground hitting his head on a foundation column. The ladder had not been secured and upset because of loose sand under the legs. The employee suffered a laceration to the back of the head that required stitches.
