WHILE DOING PILLAR RECOVERY WORK THE VICTIM WAS WAS RUNNING A REMOTE CONTINUOUS MINER WHEN A PIECE OF THE ROOF APPROX. 30 INCHX 18 FT WIDE X 80 FT LONG SLABBED OUT CATCHING THE VICTIM UNDER IT AND CAUSING IMMEDICATED DEATH THE MINER WAS ALSO COVERED UP.

WHILE DOING PILLAR RECOVERY WORK THE VICTIM WAS HELPING ON A REMOTE CONTINUOUS MINER WHEN A PIECE OF THE ROOF APPROX. 30 INCH THICK X 18 FT LONG SLABBED OUT CATCHING THE ICTIM UNDER IT AND CAU SING IMMEDICATE DEATH. THE CONTINUOUS MINER WAS ALSO COVERED UP.
