SHUTTLE CAR DRIFTED OFF BLOCKS. THE SHUTTLE CAR HAD BEEN RAISED ADN BLOCKED TO ALLOW REPAIR WORK ON THE CONVEYOR. THE VICTIM HAD POSITIONED HIMSELF UNDER THE SHUTTLE CAR TO PERFORM THE REPAI R WORK WHEN THE SHUTTLE CAR ROLLED OFF THE BLOCKING, CRUSHING HIM AGAINST THE MINE FLOOR.
