Employee states he was pulling on a suction hose of a water pump that was stuck in the mud and injured his lower back.

Employee states he was installing flashing on a conveyor belt to prevent spillage. As he was pulling on the flashing to properly place it his right hand went across a piece of jagged metal resulting in a laceration to his index finger on his right hand.

Employee states he was operating a water wagon to wet the roadways. In the evening the employees back began to bother him.

Employee was giving hand signals to a front end loader operator as they were splicing a belt. The nylon choker broke and struck employee in the chest.

Employee was helping to push a 6 inch pipe on to the bed of a truck when he slipped resulting in his hand being smashed between the bed and the pipe.

Employee states he was shoveling and felt pain in his shoulder.

Employee states he was cleaning built up material out of a catch hopper on a conveyor line with a shovel when a rock hit his hand.

EMPLOYEE WAS AT THE REAR SECTION OF AN 8" PIPE THAT WAS BEING PULLED FROM A TRENCH BY A 824 JOHN DEERE LOADER. THE REAR OF THE PIPE SUDDENLY CAME UP OUT OF THE TRENCH AND STRUCK HIM IN THE CHEST.

Employee states he was washing out the inside of the log washer. He slipped and fell onto the paddles which resulted in bruising his ribs.

Employee states he was removing a turbo from a Caterpillar generator. He states he had a wrench in his left hand and reached around the exhaust with his right hand. He was trying to put the wrench in his right hand and as he was doing this a metal gasket that joins the exhaust to manifold cut his right forearm.

Employee states he was tightening a bolt on the shaker screen and the bolt broke causing the wrench to strike him in the nose.

Employee states he was securing crane to lowboy trailer. As he was throwing chain through the hook of the crane to secure felt a pain in his right shoulder.

Employee was removing a lid off of 120lb can of grease with a screw driver. As he was removing the lid the screwdriver slipped lacerating his middle finger on his right hand.

Employee was striking a U joint with a hammer on a Terex Scraper when a piece of metal came off of the U joint entering his right index finger.

Employee states he had positioned a 6X10 sheet of metal in a vertical position in a hopper to weld. He had placed (3) 2x2's in a horizontal position with one end against the sheet of metal and the other against the wall. As he was climbing out of the hopper on a ladder sheet of metal came unsecured and pinned him against the ladder.

Employee states he was driving a hinge pin into an ejector pan with a sledge hammmer.

Employee states he was walking down stairs of control tower when he stepped on a wrench and twisted his ankle.

Employee states he was driving a groung rod with a hand held tool designed for this task, when he hit himself in the head.

Employee had been welding and helping another welder weld on a drainage pipe throughout the shift. Employee was diagnosed with flash burn.

Employee states he was climbing down the steps of the trackhoe and felt his left shoulder pop.

Employee states he was setting up 100ft piggy back conveyor in pit. He was lifting cross ties and pulling SO cable.

Employee was replacing flooring at the primary plant. The employee was using a pry bar to move a steel plate when he felt pain in his back.

Employee states he was removing hydraulic pump and it slipped and caught his finger between metal lines and cut left middle finger.

Employee was knocking the pin out of the bucket from the stick of the backhoe. The stick moved causing the pry bar to pinch between the stick and bucket. The pry bar then hit employee in his right collar bone.

Employee states he was taking compressor off of engine , had it unbolted, reached for a hammer, when the compressor fell off of mounting studs. He grabbed the compressor to keep it from falling.

Employee was getting off of a track hoe that he had loaded onto a low boy, when he slipped and fell from the tracks onto the low boy trailer.

Employee states that he had a back injury on October 9, 2006 while moving crossties at a conveyor belt. He did not report this when it happened. It was reported to us after the first of the year.

Employee hit his left ring finger with a hammer trying to knock a pin out of a roller.

Employee was opening a fuel fill cap on storage tank on truck, spring loaded lock on cap hit him in the face.

Employee was wiring up the crusher and as the loader operator was back dragging the yard something blew into the employee's eye.

Employee slipped getting down from a front end loader and his right foot got caught in a step causing him to fall backwards twisting his right leg, knee, and ankle.

ee alledgedly slipped & fell & struck a wing wall as he was climbing down from a conveyor after he removed material from the conveyor.

EMPLOYEE WAS UNHOOKING CHAINS FROM A CONVEYOR COVER THAT WAS BEING LOADED ON TO A FLATBED TRUCK. EMPLOYEE HAD UNHOOKED CHAINS AND WAS GOING TO UNHOOK OTHER CHAINS WHEN A BOOM POLE ATTACHED TO A FRONT END LOADEDER STRUCK HIM IN THE LEFT SHOUDER.

While using a grinder, employee got some metal in eye.

While changing air filter, serviceman cut right thumb on sharp metal barb.

Water pump rolled over right thumb.

While turning conveyor off he tripped on roller frame.

SHOVELING FROM UNDER THE CONVEYOR AND STRAINED HIS BACK

HEARING LOSS OF 30 TO THE LEFT EAR AT A RANGE OF 6000 HZ.

CLEANING ROCK OUT OF TAILPULLEY, PRY BAR SLIPPED AND HIS HAND HIT THE FINS IN THE PULLEY, CUTTING HIS LEFT POINTING FINGER.

Employee was putting an oxygen bottle on the service truck and dropped the bottle on his right foot.The employee received a contusion to his right foot.

RIDING A 4-WEELER ACROSS THE PIT AREA AND RAN OFF A 6 TO 8 FOOT HIGHWALL. THE IMPACT CAUSED HIM TO HIT HIS FACE ON THE HANDLE BARS.HE RRECEIVED LACERATIONS TO HIS NOSE AND FACE ALSO BRUISES TO HIS BODY.

WIRING IN FEED HOPPER & TRANSFER BELT WITH THE GENERATOR RUNNING

WHILE USING A PRYBAR TO ALIGN A ENGINE GOING INTO A BACKHOE, THE PRYBAR SLIPPED AND CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE PRYBAR AND THE ENGINE.CONTUSION AND LACERATION TO THE LEFT LITTLE FINGEER AND BROKE THE END OF FINGER.

CUTTING WELD OFF DRAGLINE GEAR. GEAR WAS OUT OF MACHINE BUT THE GEAR ITSELF WAS NOT SECURED & WHEN THE WELD WAS KNOCKED OFF THE GEAR SLIPPED OFF THE SHAFT HITTING HIM IN THE HEAD & PINNING HIM UNDER THE GEAR.

AGE CORRECTED HEARING LOSS OF 39 AT 4000 HZ IN RIGHT EAR.

AGE CORRECTED HEARING LOSS OF 43 AT 4000 H2 IN RIGHT EAR.

EE WAS TURNING PULLEY ON SAND PUMP TO START INPELLER ONTO THE PUMP AND CAUGHT HIS FINGERS OF THE RIGHT HAND BETWEEN THE PULLEY AND BELTS.

WHILE OPERATING A DOZER, HIS BACK STARTED HURTING. HE SAID THAT HE THOUGHT IT WAS DUE TO A PREVIOUS INJURY ON 3-28-02. AT THAT TIME HE WAS WORKING FOR LEHMAN-ROBERTS & WAS COVERED BY OSHA. HE TOLD THE DR. IT HAD BEEN HURTING FOR ABOUT TWO WEEKS& A NEW CLAIM WAS STARTED. REPORTED TO US ON 2-24-03.

DIGGING OUT TRACKS ON DOZER HE FELT BURNING SENSATION IN GROIN AREA.
