A haul unit was being loaded by a track hoe and a slush pond dam broke and the slush came down on top of them burying them.
A track hoe was loading a haul unit with gravel to take to wash plant and the dam on a slush pond broke and covered up both the tack hoe and the haul unit with the workers in them.
The employee was stripping at gravel pit. The employee had dumped his load of dirt and was returning to the load site. At this time he chose to travel over a pile of dirt. as he was traveling over the pile of dirt he got too close to the edge of the pile and the equipment overturned and landed on him.
Employee became entangled between a return idler and the bottom conveyor tail pulley located in the primary crusher basement.
EMPLOYEE WAS FATALLY INJURED WHEN A GRIZZLY FELL ON HIM AS HE AND A CO-WORKER WORKED TO CLEAR A HANG-UP OF MATERIAL INSIDE A HOPPER. PRIOR TO THE ACCIDENT, THEY HAD RAISED THE FRONT OF THE GRI ZZLY AND PROPPED IT ABOVE THE HOPPER SO THEY WOULD HAVE ROOM TO WORK.
EE WAS ATTEMPTING TO REMOVE IMPELLER FROM PUMP. HE HEATED IMPELLER AND A PIECE OF METAL STRUCK HIM IN CHEST.
EE ATTEMPTING TO REMOVE DEBRIS FROM AREA AROUND DREDGE BY TYPING ROPE TO TREE ON BANK. ROPE APPARENTLY GOT BENEATH BOAT AND WHEN THE ROPE BECAME TENSE, TURNED THE BOAT OVER. TWO EE FELL INTO W ATER AND ONE WAS NOT RECOVERED.
EE WAS ON DUTY ON THE DREDGE AND EVIDENTLY FELL IN RIVER FOR UNKNOWN REASONS.
VICTIM WAS WALKING PIPELINE FROM SHORE TO DREDGESLIPPED AND FELL INTO THE WATER AND DROWNED. WASNOT WEARING LIFR JACKET.
DREDGE OPERATOR WAS ATTEMPTING TO DISLODGE SAND ROCK FROM SUCTION WHEN HE FELL OVERBOARD FROM SKIFF NO WITNESSES ACTUALLY SAW HIM FALL OUT HE WAS NOT WEARING HIS LIFE JACKET THAT WAS PROVIDED.
EMPLOYEE WAS WALKING UNDER AN ELEVATED CONVEYOR BELT WHEN SUPPORT CABLE BROKE DROPPING CONVEYOR ON EMPLOYEE.CONVEYOR HAD BEEN TAKEN OUT OF SERVICE AND WAS NOT OPERABLE AT THE TIME.BROKEN LEG C
EMPLOYEE WAS HAULING GRAVEL TO RAMP LEADING TO THE PIT AREA. APPARENTLY EMPLOYEE BEGAN TO DRIVE DOWN THE RAMP AND WAS UNABLE TO BRING TRUCK TO A STOPPED POSITION. HE OPENED THE DRIVER'S DOOR A ND JUMPED FROM THE TRUCK. THE TRUCK ROLLED OVER ON THE DRIVER'S SIDE AND ONTO THE OPERATOR. MUSHA WAS IMMEDIATELY NOTIFIED OF THE ACCIDENT BY THE CORPORATE SAFETY DIRECTOR.
