The EE was bolting on Main West Haulage at 106 block. While EE was changing steels a rock fell from the mine roof and struck the EE on top the head. EE finished EE's shift, and went for medical treatment the next day.
The employee was center bolting in the #3 entry of the 1 West Head Gate section. While changing drill bits a piece of rock fell from the top striking employees hard hat and then right hand. The employee has a strain/sprain in EE's neck and middle finger. Employee was wearing all PPE.

The injured was bolting on the 1WTG Section. EE was reaching across the miner to get a strap, when a piece of roof material fell out and struck EE in the back of the head causing injury. The employee did not violate any laws or company polices. EE was also wearing all appropriate PPE.
The employee was putting a strap across the miner in the 1WTG Section, when a piece of roof material fell out striking head, arm, and leg. The employee continued to work until they realized arm was bleeding. The laceration sustained on left arm was glued back together. EE didn't violate any laws or company policies and was wearing the appropriate PPE.
Employee was walking down the #4 mainline belt, and had stopped to look underneath the belt at 20blk. EE placed hand on the bed rail while looking under the belt, and coal fell out from the mine roof and landed on EE's right ring finger, causing a contusion on the finger and hand. Employee was complying with all laws, company policies and was wearing all appropriate PPE.
The Employee was on the 1 West Tailgate section at 70blk #2 entry, in the process of installing a rib bolt from the miner. Loose material fell from the mine roof onto the ventilation tubing and then slid into the side of EE's head above the ear causing a laceration. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.
The Employee was on the 1 West tailgate section at 70blk #1 entry, in the process of installing roofbolts from the miner. Loose material fell from the mine roof striking the employee in the upper right leg, causing EE's hip to twist and strain. The Employee was complying with all laws, company policies, and was wearing all appropriate PPE.
The Employee was on the 1 West Tailgate Section walking by the miner, to the bolter station. As the employee was walking passed the miner, loose material fell from the mine roof, striking the right side of upper back, causing minor cuts and a contusion. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.
The employee was rib bolting on the 1WHG Section. The employee scaled the mine roof, and was closing the rib drill. Additional material fell out of the mine roof striking the employee's left hand lacerating the pinky finger. The employee was wearing all the appropriate PPE and was obeying all laws and company policies.
The employee was roof bolting off of the continuous miner when material fell from the mine roof and stuck the employee on the right side of the head, neck and shoulder causing pain. The employee was wearing all the appropriate PPE and did not violate any laws or company policies.

The employee went to seek medical attention 5/1/19 and was prescribed medical eye drops and given time off. The employee was wearing all the appropriate PPE.

As the employee was in the process of drilling a bolt hole, a Piece of head coal fell from the mine roof striking the employee in the head, straining neck. The employee was working on 1WHG Section, helping to hang a tube in the #2 recovery chute. A piece of rock/coal fell out of the mine roof and struck the employee in the mouth. The employee received four stitches in EE's gum. The employee did not violate any laws or company polices and was properly trained.
The employee was bolting on the left side of the miner, when material fell out of the roof and struck the employee on the left shoulder and lower arm. The employee was wearing all appropriate PPE.
While bolting in #1 entry of 6E with a center bolter, the drill steel got stuck in the roof. The injured reached out to retrieve the steel and a rock fell out of the top, striking left upper arm and shoulder area, resulting in contusions.
Bolting off right side of miner in St Leo while cutting a corner in 3-4 crosscut. Injured had installed a roof channel and was walking toward the back of the miner when a rock fell from between the straps (4'x2'x4" thick) striking the top of their head and knocking them into the mining machine, resulting in a lost tooth and minor abrasions.
After installing bolt off miner in 6E #3 entry, just outby 9 crosscut, the injured was walking toward the tail of the miner when a piece of rock fell from the roof and struck head. When ee reported to work on 5/30/18 ee reported the incident and went to seek medical attention, being diagnosed with a concussion.
While mining 4-5 crosscut at 97 block St Leo the return side bolter operator, on an ABM14, had hand on top of rib door while attempting to extend the slider ventilation tube with right hand when a rock fell from behind the tube and struck the posterior portion of wrist and metacarpal area of gloved left hand, resulting in a contusion.
Injured was retrieving a cap board near the section power car in St Leo when rock and coal fell out of the roof. The injured turned and ran, but a piece of rock/coal struck EE's left knee resulting in a sprain/strain injury from the impact of the falling rock combined with twisting at the knee to run. Employee did not start missing work until 2/21/18.
Injured was the rib bolter on the right side of the full face Alpine miner in the face of the future belt entry in St Leo when some head coal fell out from between the roof straps (approx. 4' long, 5' wide and 2-3" thick) and struck the injured's hard hat, driving it down and causing minor lacerations (no sutures) to nose and lip, as well as a minor contusion behind right ear.
Injured was attempting to hang miner cable slack (between miner and loader) in 1 left to 2 left at 5 block of 1EHG longwall setup development section. A piece of draw rock slid off a roof strap (2' diameter, 1/4-1" thick) struck injured's hard hat, went down their body resulting in minor contusions and abrasions, most severe injury was a sprained ankle.
While bolting in St Leo construction area (dual head fletcher), EE was retrieving bolt supplies off the bolter supply area a rock (10-14") fell out and struck EE's head, resulting in a soft tissue strain to cervical spine area. No problems with mining methods, PPE, training or compliance issues. Injured has previous history of neck problems.
Profiling 15 belt at 346 block with a come-a-long attached from the structure to a belt hanger. Head coal (20"x10"x5") fell out from around the belt hanger and hit EE in the hard hat, knocking EE down. Being evaluated for a concussion. No problems with mining machines, PPE, training, or compliance issues.
