EE was moving miner cable out of his way when his hard hat fell off. While he was getting his hard hat a piece of rock-coal fell hitting him on the head.
While bolting top, a piece of cap coal/slate approx 18" x 24" x 3" fell out striking employee on left forearm and left upper leg.
Victim was cleaning on #5 belt. Piece of rock fell from between roof bolts striking victim in the head.
EE was operating roof bolter when a piece of rock fell out between bolter. The rock hit the EE on the side of his head and neck.
The injured was working on #3 SC in the #6 entry, when a rock 1'x8"x4" fell from the top, sliding down the right shoulder and right side of body. Bruised the right index finger and striking the right leg. No injuries other than the bruised finger and sore right leg were found.
#7 & 8 crosscut, went to drill hole & piece of cap coal hit him in back of head & neck.
He was bolting top; a rock came down drill steel, hit him on shoulder and right foot.
Miner operator sounded roof and ask bolter operator if he would spot bolt the area for additional support. While spot bolting rock fell and landed on operators right heel.
EE WAS STANDING AT THE REAR OF THE ROOF BOLTER WHEN CAP COAL AND ROCK FELL, STRIKING THE BOLTER AND SLID OFF AND STRUCK LEFT FOOT CUTTING AND MASHING 4 TOES. EE WAS SENT TO RALEIGH GEN. ROCK WAS APPROX 3' WIDE, 4' LONG, RANGING FROM 18" UP TO 4" THICK. TRANSFERRED TO MORGANTOWN TO SEE SPECIALIST.
