EE was on the left side of the No. 6 entry approx. 10 ft. outby the last open crosscut performing repairs to the offside of the continuous miner when a piece of draw rock measuring approx. 4' L x 28" W & 4-5" thick fell striking EE on left shoulder. A large lump immediately popped up on EE's left shoulder. EE was transported to the surface by rail mounted vehicle.
Was bolting in #4 Right when a piece of draw rock fell striking EE on the right arm and cutting EE requiring 3 stitches.
When the drill bit contacted the immediate roof a layer of draw rock measuring 17 inches X 17 inches x feather edge to 1 inch thick, fell striking EE on EE's right thumb and forefinger resulting in 2 broken bones and 1 fractured bone.
During the process of inserting a roof bolt through a steel strap into the drilled hole a small piece of draw rock was dislodged and struck EE on EE's left forearm resulting in a laceration.
EE was installing roof bolts and steel straps a piece of draw rock dropped out in the area of the T-bar, it hit the loose end of the straps sliding down the straps to the mine floor. When the rock measuring 4'x4'x3" thick contacted the mine floor flipped back on to ee right leg resulting in a bulging fracture to the ankle area.
Employee was installing roof bolts in the face of the No. 2 entry. After adding the finishing piece of steel in place he placed his hand on the drill steel until it was fully inserted back into the hole, at which time the piece of rock measuring 15-16 in. wide, 78 in. long & 2-2 1/2 in. thick fell next to the steel striking him in the area of the 4th-5th fingers of the left hand.
EE was located in a intersection off the left inby corner observing the timber line while retreat mining was being performed. A section of rock and coal rolled off the corner measuring 24"H x 8"W x 8"T, striking ee in the lower back. The ribs were soft due to the retract mining.
A PIECE OF DRAW ROCK HIT EMPLOYEE BETWEEN THE SHOULDER BLADES, BRUISING HIS BACK. ROCK WEIGHED ABOUT 3 TO 5 POUNDS.
EMPLOYEE WAS SITTING ON A PORT-A-LET EATING HIS DINNER WHEN A PIECE OF DRAW ROCK, 1' X 2' X 4" FELL ONTO HIS KNEE.
EMPLOYEE WAS WALKING BETWEEN GENERAL LABOR JOBS,WHERE A ROCK FELL, STRIKING HIS RIGHT FOOT, DAMAGING CARTILAGE AND BREAKING HIS RIGHT BIG TOE.
EE WAS ROCK DUSTING THE LEFT RIB IN #2 ENTRY WHEN A PIECE OF DRAW ROCK FELL FROM THE TOP STRIKING HIM IN THE LEFT SHOULDER HE COMPLAINED OF SEVERE PAIN IN HIS SHOULDER AND ARM.
EMP WAS SHOVELING BELT WHEN A PIECE OF DRAW ROCKFELL HITTING EMP IN RIGHT SHOULDER. SHOULDER WASVISIBLY BRUISED AND SCRATCHED. EMP WAS ABLE TO MOVE ARM FROM SIDE TO SIDE BUT COMPLAINED OF PAIN IN SHOULDER.
